The Neighborhoods at St. Elizabeth is currently seeking a Licensed Practical Nurse who is responsible for the effective overall management of clinical services and for providing quality nursing care to all program participants. Responsibility lies in coordinating three major areas: care of program participants, administration of medication and treatment, and clinical assessments of program participants.
The Neighborhoods at St. Elizabeth Rehabilitation and Nursing Center provides a comprehensive array of individualized rehabilitation and nursing services, including short-term rehabilitation, skilled nursing care, memory care and in-patient hospice care.
JOB DUTIES & RESPONSIBILITIES:
- Administers direct nursing care to selected program participants to maintain clinical competence and/or to assess nursing care needs of program participants.
- Works collaboratively with staff, peers and other health care professionals to ensure effective clinical management of elder care practices.
- Follows and reviews nursing care plans for program participants on an on-going basis.
- Maintains necessary documentation per policies and procedures and as required by State and Federal regulations.
- Obtains and records vital signs.
- Adheres to Infection Control and environmental safety procedures.
- Delegates tasks to appropriate staff members to ensure effective functioning of the unit.
- Ensures medication is administered as ordered.
- Ensures treatments are completed as ordered.
- Provides supervision and/or resources to Neighborhood Nurses in other Neighborhoods as necessary.
- Assists new staff members in the adjustment and orientation process.
- Participates in the in the evaluation process of nursing staff.
- Maintains adequate supplies on the Neighborhood and contains costs.
- Serves as a liaison between the Neighborhood and other departments in advocating the needs of program participants.
EDUCATION & EXPERIENCE REQUIREMENTS:
- Graduate from a State approved school of nursing. Licensed as a Licensed Practical Nurse in the State of Maryland.
- Clinical experience in long-term care and with the dementia population.
Certification/Licensure:
- LPN
- CPR

- Excellent organizational skills and attention to detail.
- Basic knowledge of clerical procedures and systems such as managing Outlook calendar, email and recordkeeping.
PHYSICAL REQUIREMENTS & WORK ENVIRONMENT:
- This position requires the ability to use full vision, hearing, complete range of motion and the ability to lift at least 75 lbs.
- Hearing and vision abilities within normal range (corrected) to observe and communicate with others.
- Moving about to accomplish tasks or moving from one work site to another.
