Reports to: Diabetes Program Manager
Worksite: Leech Lake Diabetes Center/Outreach Clinics
Work Hours: 8:00am to 4:30pm, 3-5 days/week, occasional evenings and weekends

License/Certification Requirements: Must be a Registered Dietitian licensed in the State of Minnesota. Must have a valid MN driver's license, current insurance or be insurable, CPR and first aid certification.
Summary: Provide expert Medical Nutrition Therapy, Diabetes Self-Management Education and consultation to the Leech Lake Reservation. Raise awareness of healthy nutrition on the Leech Lake Reservation to the highest level possible by managing or preventing nutrition-related diseases with a strong focus on pre-diabetes, diabetes, heart disease, obesity and cancer.
Education Requirements and Experience:
Bachelor's degree in Dietetics, Human Nutrition, Nutrition Education, Food and Nutrition, or Food Services Management required.
Must be a Registered Dietitian licensed in the State of Minnesota.
Certified Diabetes Care and Education Specialist (CDCES) preferred or willingness to work toward.
Additional Qualifications and Skills:
Excellent time management, organization, conflict resolution, public relations and speaking skills are required.
Ability to establish and maintain an effective working relationship with staff, clients, and other professionals
Ability to effectively and efficiently communicate, both verbally and in writing is required
All licensing renewal requirements as specified by the MN State Board of Dietetics and Nutrition Practice or as required by the state they are licensed in, including required CEU's
Basic understanding of Diabetes; e.g., path physiology, treatment options, health promotion, and disease prevention Experience and knowledge of Diabetes patient education principals and methods Experience in utilization of Electronic Health Record (EHR) technology. Native American preference

Duties/Responsibilities:
Provide nutrition consultation in tribal health programs, diabetes program, elderly nutrition program, Head -Start programs, day care centers, Tribal schools, food programs and WIC programs as it relates to diabetes prevention and management.
Assist with menu development for programs seeking diabetes management and prevention related menus. Will provide comprehensive and general Medical Nutrition Therapy, education and care to patients, their families, and the community in multiple settings; e.g., clinics, hospital, homes, community centers, and schools. Aids with case management of newly diagnosed patients and patients utilizing staged diabetes management per referral.
Collaborates with physicians regarding patient's plan of treatment.
Will maintain appropriate program records in compliance with IHS, Tribal, and granting agency policies
Reviews, develops, and selects appropriate educational materials to be used with clients and for use in training health care professionals and para professionals.
Collaborates with other community health resources; e.g., IHS, other Tribal Health programs, and community agencies.
Promotes diabetes related health and wellness through community based educational activities and classes.
Assists with program planning, development, implementation, and evaluation.
Will disseminate diabetes related program issues and experiences to appropriate agencies and individuals. Assists with diabetes related medical record, reviews, audits, documentation, data collection, and quality improvement.
Maintain confidentiality and professional ethics at all times.
