(569) Intensive Case Manager - Adult Services

The person in this position is responsible for the delivery of intensive case management and psychosocial rehabilitation services to people with severe and persistent mental illnesses, as defined by the state authority.  This position provides uniform assessments required by the state authority and the behavioral health center, collaborates with individuals served to develop person-centered recovery plans, and provides rehabilitation skills-training and documentation of services delivered in accordance with individual need and Medicaid standards. The person in this position participates in Quality Improvement activities to contribute to a culture of continuous quality improvement in the behavioral health center.

The person in this position should provide training in skills that primarily focus on furthering an individual’s independent living, community skills and overall wellness and recovery.  Services provided by the person in this position should promote community integration, increased stability within the community as well as an improved quality of life and implementation of self-determination for individuals served, as appropriate.  Services provided by this position also include activities and skills-training designed to address barriers and strengths related to an individual’s recovery and wellness.  Services provided in this position would include assisting an individual in coordinating access to necessary care and services appropriate to the clinical needs.  The person in this position may also be responsible for providing crisis intervention services as needed.  This position’s job tasks are provided primarily in the community, or “in vivo”, but does not exclude the need for some services to be provided in the clinic.

The person in this position requires an aptitude for working with people who have diagnoses of bipolar disorder, major depressive disorder as well as schizophrenia, schizoaffective disorder and other psychotic disorders.  This position also requires knowledge of psychotherapeutic medications as well as evidence-based practices and contraindications in the promotion of recovery for individuals with severe and persistent mental illnesses.  This position requires knowledge and skills in assessment, collaborative person-centered recovery, differential diagnosis as well as wellness and recovery-oriented systems of care.

Salary: $21.46 - $28.71/hr

Applicable Differentials:

$1.00/hour – Master’s Degree

.75/hour – Bilingual

.50/hour – LCDC

$1.00/hour – Advanced Licensure

$1.00/hr 1 year with BTCS
$1.00/hr 2 years with BTCS
$1.00/hr 4 years with BTCS
$1.00/hr 6 years with BTCS

Minimum Qualifications: A Bachelor’s degree from an accredited college or university with a major in a social, behavioral, or human service field or any bachelor’s degree with course work equivalent to a major in a social, behavioral, or human service field. Knowledge of mental illness as defined in the DSM-5 is required. Must have valid Texas Driver’s License and maintain approved driver’s status throughout employment.

Preferred Qualifications: Master’s degree and/or clinical license in psychology, education, social work or a related human service field; one-year work or practicum/clinical experience in case management, crisis intervention and/or psychosocial rehabilitative services; One year of experience in mental health assessment and/or crisis intervention and screening; Master’s degree in psychology, social work or a related human service field. Bilingual preferred; experience in providing concurrent documentation within an electronic database preferred.
