Licenses Practical Nurse/Service Coordinator

The Licenses Practical Nurse/Service Coordinator is an integral part of the Pathways to Housing team, providing flexible, community based services designed to promote recovery, stability and community integration of formerly homeless individuals who have a mental illness and/or substance abuse issues.

Duties include collaborating with teams to coordinate services, complete initial and annual nursing assessments for assigned participants, and make regular home visits to them. Monitor vital signs, provide wound care as needed, work with team and client to follow treatment plans. Be a health care resource to clients, their families/significant others, and staff. Document all services  provided in compliance with all regulatory requirements.  Collaborate with on-staff psychiatrists and other health care professionals in developing/delivering health nutrition and fitness initiatives. Participate in facilitating crisis interventions, referrals and hospitalizations as needed. Perform other duties as assigned.
