Care Manager
Summary of Position
The Care Manager position is an expert in the field of care management. Care Managers educate and guide Individuals as they make choices about their desired goals, and efficiently coordinate services that support those goals. Care Managers ensure coordination of services that result in the betterment of the lives that waiver program participants choose for themselves and acting as an advocate to ensure Individual rights and wishes are respected, while upholding the Core Values and Mission of IPMG.
Essential Functions & Responsibilities

- Care Management for those on the A & D and TBI Waiver
- Facilitate the assessment process utilizing person-centered discovery tools
- Development and implementation of a Person-Centered Support Plan
- Administering the Person-Centered Monitoring Tool
- Evaluating effectives of services
- Monitoring progress of identified needs, meeting goals and preferences
- Direct collaboration and coordination with providers
- Adjusting action and service plans to meet participant needs
- Complete annual person=centered assessments including eligibility and service planning
- Coordinating changes in service plans
- Complete and maintain all required documentation

Supervisory Responsibilities
This position does not supervise direct reports.
Required Education & Experience
- Bachelor’s degree in Psychology, Sociology, Social Work, Special Education, Nursing, Gerontology, Counseling, Rehabilitation, or other human services field
- Minimum of 1-year direct experience working with persons who are aged and/or disabled
- Knowledge of web-based systems
- Strong technology skills
- Team Facilitation experience
Preferred Qualifications & Experience
- Knowledge of waiver case management processes, standards, and regulations
- Ability to perform in a virtual environment under minimal direct supervision
Additional Eligibility Requirements
- Authorization to access the State of Indiana’s operating systems
- CPR Certification
- Current and valid driver’s license
- Current and valid car insurance
- Ability to pass a thorough background screen
Work Environment
This position functions in a home office setting, using standard office equipment such as a computer, phone, and scanner.
Physical Demands
Minimal lifting & reaching is required. The ability to physically enter Individuals’ homes and other community settings is required.
Position Type & Expected Hours of Work
This is a full-time position, working Monday-Friday, 40 hours per week, with the ability to maintain a flexible schedule to accommodate the needs of the Individuals served. All schedule changes outside a workweek of Monday – Friday or an accommodation for Individuals served, must be approved in advance by management.
Travel
Significant local travel is required (50% travel, up to 1-hour drive from home office)
