What We’re Looking For
Trillium Health Resources has a career opening for a Supervisor, Care Management to join our Regional Care Team. This position plays a key advisory role on the Care Management Team, providing support and oversight to Care Managers and their direct engagement with members. The Supervisor, Care Management is responsible for providing guidance, reinforcement, and quality control for the care team and its services across physical health, behavioral health, pharmacy, long-term services and support (LTSS), and unmet health related resource needs. In addition, this position may collaborate with or consult with intellectual/developmental disabilities (I/DD) and traumatic brain injury (TBI) teams for complex members.
On a typical day, you might:
- Ensure the care team is providing services in compliance with federal and state requirements and Trillium service requirements.
- Review care plans for appropriateness and quality and provide guidance on best practices and improvements to meet members’ specific health and social needs.
- Monitor care management quality and compliance, taking corrective action if needed.
- Facilitate learning and development of staff through initial and ongoing training as Tailored Care Management evolves and novel clinical practices emerge.

Qualifications:
Education: Master’s degree required. Prefer degree in Human Services field
Experience: A minimum of three (3) years of experience providing care management, case management, or care coordination to the MH/SU/BH population.
License/Certification: NC fully licensed LCSW, LCMHC, LPA, LMFT, LCAS or RN required.
Must have a valid driver’s license
Must reside in NC to be considered for remote status
