Care Coordination Intern - Work U
The HCO Care Coordination team is responsible for supporting the most vulnerable individuals across the state of Idaho; those who receive both Medicare and Medicaid. The team is responsible for partnering with members, providers, and informal supports to ensure individuals have access to health benefits, access to community resources, and the support they need to achieve their health and wellness goals.
Opportunity/Project Duties:
- Project 1: Build statewide network of contractors to support requests for Environmental Accessibility Adaptations. An Environmental Accessibility Adaptation (EAA) is an available Medicaid benefit for qualified members. EAA include minor housing adaptations that are necessary to enable a member to function with greater independence in the home. Adaptations may include: installation of ramps and lifts, widening doorways, modifying bathrooms, or installing electric or plumbing systems. Currently we do not have enough in-network contractors to support member requests.
- Project 2: Improve solicitation of member satisfaction. HCO Care Coordination is looking to improve HOW and WHEN we are obtaining member feedback. Currently, we are sending surveys to members 45-60 days after an assessment, but the team has many points of contact with members outside of an assessment. The team would like to explore how to capture member experience and feedback throughout their health journey with care coordination.
