Care Manager Services, RN - Internal Medicine
Kaua'i Medical Clinic is the island’s largest clinic and only multispecialty medical group. More than 85 physicians and midlevel providers offer primary and specialty care at clinic locations in Eleele, Kapaa, Koloa and Lihue.
Internal Medicine physicians specialize in the diagnosis, management and nonsurgical treatment of unusual or serious diseases. Our specialists in internal medicine or “Internists,” have a lengthy clinical and scientific training in their areas of medical interest and special expertise in management of patients with complex, interacting medical conditions. Services include immunization, preventive care and patient education through a long-term personal relationship with their patients. Our caring and professional team also coordinates the efforts of other medical and surgical specialists as needed to assure our patients’ best possible health.
As the Care Management Services Registered Nurse, you will provide care management and coordination for patients enrolled in the Care Management Program in the Primary Care setting, in partnership with the primary care practice health. You will serve in an expanded health care role to collaborate with specialists, members of the health care team and patients, families and caregivers, to ensure the delivery of quality, efficient and cost effective health care services. Minimum Qualifications: Must meet education requirements for Hawai‘i state licensing. Current Hawai‘i RN license. Bachelor’s Degree in Nursing or related field. Current American Heart Association Health Care Provider Basic Life Support (BLS) CPR card. Current driver’s license and valid Hawai‘i auto insurance with short notice access to an automobile to conduct site visits, as required. Two (2) years of clinic experience in a Primary Care setting. Experience working within a collaborative interdisciplinary team.
Preferred Qualifications: Certification in Case Management. Experience in care coordination for patients. Care management experience. Experience as participant in continuous quality improvement. Working knowledge of EMR (Electronic Medical Record).
