At Ronald McDonald House New York, AmeriCorps members will build the infrastructure needed to advance health equity outcomes for underserved medically complex children and their families receiving care at public and safety-net hospitals in New York City. AmeriCorps members will do this by canvassing communities to conduct risk assessments and market analyses to identify social determinants of health of target communities, establish referral networks with community-based organizations, and refer medically complex families being served by Ronald McDonald House New York to other services at other nonprofits to address various social determinants of health. By participating, AmeriCorps members are helping build the knowledge surrounding what services help families with sick children, and how social determinants of health impact families' access to critical care.

Member Duties : AmeriCorps Members will work with our team to: Go into target communities/hospitals to conduct social risk assessment by administering a survey (RMH-NY Social Determinants of Health (SDoH) screening) to identify SDoH impacting target communities. E.g. Hospital Outreach Conduct market research and community canvassing to build knowledge and a list of recommended Community Based Organizations (CBOs) that can provide services to vulnerable families with medically complex children e.g. Childcare, food pantries, immigration application assistance, etc. Conduct outreach to selected nonprofit agencies to establish referral relationships with RMH-NY for SDoH service provision e.g. Childcare, food pantries, Transporation services. Work with Care Coordinators from RMH-NY to provide education to staff and other organizations, and coordinate SDoH services for families.
