AmeriCorps Community Health Worker

Overview
Are you looking for experience in the Public Health field? The Larimer County Department of Health and Environment (LCDHE) has an exciting opportunity for you!

In partnership with Colorado Public Health Works and AmeriCorps, we are offering a paid apprenticeship program to further your Public Health knowledge and skills and help you gain the experience you need to start your career in Public Health.

As an AmeriCorps Community Health Worker (CHW), you will be a frontline public health connector. By building trusting relationships with the community you will serve as a liaison between health/social services and the community, facilitate access to services, and improve the quality and cultural competence of service delivery.

Responsibilities
Specifically, this position:
  • Coordinates care, provides case management support, and assists individuals and communities in navigating health and social service systems.
  • Performs cultural mediation among individuals, communities, and health and social service systems.
  • Provides culturally appropriate health education and information.
  • Provides coaching and social support.
  • Assists with promotion for individuals and communities.
  • Builds individual and community capacity and growth through a range of activities such as outreach, community education, informal counseling, social support and advocacy.
  • Implements individual and community assessments.
  • Completes an average of three hours of training each week, provided primarily in a virtual environment, with some limited in-person trainings. Each host site will provide on the job training with appropriate mentoring and supervision for the CHW.

We are looking for a candidate that will:
  • Have both lived or professional experience with underserved communities including but not limited to Latinx, Black/African American, Native American/Indigenous, LGBTQ+, housing unstable, and/or community members living below the Federal Poverty level.
  • Have foundational knowledge of one or more of these populations, and will be able to use their communication and interpersonal skills to connect and build trust with community members across Larimer County.
  • Work directly with members of the community, professional partners, AmeriCorps staff and LCDHE’s staff in goal setting, establishing priorities for work, and connecting them with other community members, organizations, and staff from other departments as needed to accomplish those priorities.
  • Demonstrate innovation, collaboration, and strong interpersonal skills.

Qualifications
All candidates will be screened for the following required minimum qualifications. Any candidate who does not meet all of the qualifications listed as required will be eliminated from the process.
  • Sophomore level course work in Environmental Health, Public Health, or a related field required.
  • Possession of, or ability to obtain, a valid Driver's License required.
  • Bilingual (English/Spanish) skills preferred.

Supplemental Information
This is a temporary position. The typical schedule for this position is 8am - 4:30pm Monday to Friday, but occasional weekend and evening hours my be needed. 

Member Terms: 
  • Members serve for 1 full year (12 months), 40 hours per week with 40 hours vacation/time off permitted.
  • 2000 hours are completed to meet the requirement for the AmeriCorps and Apprenticeship model and includes comprehensive training and direct service-learning activities. Participants may be eligible to apply previous hours for experience and education to the term.

Other Benefits
  • Post service Education Award, for successful completion of term up to $6,483 to apply towards tuition at eligible institutions. 

An offer of employment is contingent upon the successful completion of a pre-employment drug screen and background check. As marijuana is an illegal substance under federal law, testing positive for marijuana (medical or otherwise) or any other substance for which the county tests, will result in the contingent offer of employment being revoked. A successful completion of the drug screen is defined as obtaining a negative result.
