Care Transition Coordinator

Community Care Network is seeking a Care Transition Coordinator in Rutland, Vermont.
Community Care Network is comprised of Rutland Mental Health Services and Rutland Community Programs. Our mission is to enhance the well-being of our communities, individuals and families through responsive, innovative and collaborative human services.
About the Role:
Certified Community Behavioral Health Centers (CCBHCs) are required to collect a significant amount of data from clients upon entry into services and at select intervals, as directed by the Government Performance and Results Act (GPRA).
The Care Transition Coordinator conducts client interviews via telephone, online interface, or face-to-face, gathering demographics, clinical, social, medical, financial, and other information. Data gathered is used for State, Federal, and internal reporting, with a focus on Continuous Quality Improvement (CQI). Interviews conducted are central to reducing behavioral health disparities by identifying social determinants of health. The Care Transition Coordinator maintains close working relationships with administrative and clinical teams in order to provide coordinated, high quality services. This position is a liaison for new and existing families, individuals and teams until long term supports are engaged and maintained.
Principal Responsibilities:
  • Coordinate transitional care for those entering services.
  • Conduct structured interviews with adults and adolescents accessing treatment for substance use and/or mental health disorders and warm hand-offs to clinical teams.
  • Data management, tracking, and reporting (GPRA).
  • Administer standardized assessments tools and enter responses into data systems.
  • Work with healthcare data and electronic health records where confidentiality is paramount.
  • Referral to community supports.
Qualifications and Competencies:
  • Bachelor's degree in related field preferred.
  • Experience managing behavioral health data and serving people with mental illness and/or substance use disorder preferred.
  • Ability to receive and convey information in efficient and effective manner to facilitate impactful screenings. 
  • Proficiency with computers; experience entering data into electronic health records program.
  • Ability to maintain confidentiality.
  • Flexibility, dependability, and ability to work independently.
  • Excellent interpersonal and organizational skills.
  • Effective communication skills, both written and verbal.
  • Ability to maintain a calm and professional demeanor during stressful situations.
  • Must have a valid driver's license, reliable transportation with personal automobile liability insurance coverage at or above the levels mandated by the State of Vermont.
  • Successful completion of a criminal history background check after hire.
