Social Worker, 35hrs/week, flexible hours Monday-Friday

JOB OVERVIEW
  • Responsible for the collaborative team process which evaluates, plans, implements, coordinates, educates and monitors the options and services to meet the patient’s health needs, using communication and available resources to promote high quality, cost effective outcomes.
  • Manages outpatient cases to ensure the patient rights are upheld. Implements clinically appropriate discharge plans. 
  • Advocates for patients in availing themselves of community social, medical and financial resources. 
  • Ensures that patients who are discharged from Madonna’s specialty programs maximize opportunities for ongoing follow-up assessments by assisting them with resource needs necessary to complete the visit.
  • Coordinates multi-disciplinary worker’s compensation cases and provides ongoing communication with employers/case managers/adjusters as well as provides insurance verification and obtains authorizations for such cases.
  • Work will be performed in an ethical and legal manner following organizational policies, processes, protocols and procedures.
  • Responsible for quality service delivery and internal/external customer relations for Madonna as a whole, including upholding the mission and values for the department and facility.

ESSENTIAL FUNCTIONS
(INFORMAITON)
  1. Analyze patient’s past and current status, assess impairments and disabilities in order to plan, coordinate, implement and monitor an effective and efficient plan of care.
  2. Analyze and monitor patient’s need for equipment, transportation, housing, caregiver services, etc., in order to coordinate the provision of and payment for such services.
  3. Monitor the patient’s current status as well as the information in the medical record to evaluate and ensure the appropriate provision of care.
  4. Analyze, assess and monitor patient’s financial information through the use of Affinity and/or other sources in order to maximize appropriate utilization of resources.
  5. Compile, coordinate and analyze patient’s program information in accordance with the requirements of third party payers in order to maximize appropriate utilization of patient’s financial benefits.
  6. Compile and analyze information to monitor patient’s response to the current treatment plan, assess available clinically and financially appropriate alternatives in order to plan and implement appropriate placement in the continuum of care.
  7. Coordinate and compile patient information in order to ensure that discharge arrangements are completed and facilitate the implementation for discharge recommendations.
  8. Compile, coordinate and analyze patient medical data to provide for continuity of care and meet individual patient/family needs as they relate to the outpatient program.
  9. Compile and process patient financial assistance applications.

(PEOPLE)
  1. Orient patients and families to outpatient services, patient rights, admission and discharge criteria and patient/family responsibilities in order to ensure patient/family active participation in the program.
  2. Collaborate with patients and families to ensure patient/family involvement in treatment planning, discharge planning and aftercare services.
  3. Collaborate with patient/families in order to obtain information regarding previous functioning to facilitate plan of care.
  4. Communicate with patients and families regarding financial information in order to maximize informed decision making.
  5. Collaborate and communicate with patients, families and other customers regarding concerns/grievances and facilitate resolution in order to ensure customer satisfaction.
  6. Collaborate and consult with community agencies regarding patient needs and agency ability to meet needs, making appropriate referrals and arrangements to provide for continuity of care.
  7. Communicate with patients and family members regarding advanced directives and legal protection in order to enhance patient self-determination and protection.
  8. Collaborate with clinical team regarding availability of services, patient preferences and patient rights in order to develop a plan of care and a discharge plan.
  9. Mentor, communicate and collaborate with members of the interdisciplinary team to monitor and resolve issues related to the plan of care in order to maximize customer satisfaction, patient/family participation and efficient, effective care delivery.
  10. Collaborate and communicate with interdisciplinary team and patient financial services regarding patient’s financial information in order to maximize patient outcomes and responsible utilization of resources.
  11. Collaborate and communicate with internal and external physicians and other service providers regarding patient’s progress, needs and program goals in order to coordinate and evaluate the program to maximize patient outcomes and responsible utilization of resources.
  12. Negotiate and communicate with third party payer representatives in order to develop single case agreements for services when necessary.
  13. Collaborate and communicate with external case managers regarding patient’s clinical program in order to evaluate the program, maximize patient’s outcomes, and appropriate utilization of resources.
  14. Collaborate and communicate with interdisciplinary team and other stakeholders in order to ensure that discharge arrangements are complete and facilitate the implementation of discharge recommendations.
  15. Collaborate and communicate with team members, patients and families, and other stakeholders to coordinate follow-up in order to ensure customer satisfaction
  16. Communicate with supervisor and other appropriate staff regarding difficult case situations and/or interdisciplinary team difficulties in order to assess, monitor and evaluate quality service delivery.
  17. Follow instruction from supervisor to perform other functions as assigned in order to achieve the goals within the department and/or system.

QUALIFICATIONS (Education/training and/or Experience)
Education
  • Bachelor’s Degree in Social Work from an accredited school of social work required.
  • Must possess current CWS (Certified Social Worker) certification or obtain certification within the first three months of employment.
  • Must have a minimum of 1 year of clinical experience in physical rehabilitation. 
  • General knowledge of Medicare and Medicaid programs and basic knowledge of Worker’s Compensation, indemnity/liability, managed care and private insurance required. 
  • Knowledge of general rehabilitation principles, community resources and group dynamics required. 
  • Must possess effective skills in collaboration, group facilitation, conflict management, and consensus building. 
  • Must possess well developed written and verbal communication, judgment and program solving skills. 
Experience
  • Experience in medical case management or team facilitation preferred. 
  • Experience with the elderly and/or disabled preferred.

Current driver’s license and good driving record required. Must submit a copy of current driver’s license abstract reflecting a good driving record.
