Resources and Services Navigator

This is a full-time benefitted position located in our Twin Falls office. This position will be able to hybrid telecommute after probation.

Our generous benefits package increases the total compensation of a full-time (benefits eligible) Resources and Services Navigator making $20.82/hour to an estimated $31.31/hour.

The Idaho Department of Health & Welfare is a drug-free workplace. Example of Duties

  • Identify individual and family strengths and service needs through skillful interviewing and discussion with participants, and review of enrollment forms and requests for service. 
  • Provide appropriate information, referral and diversion to community resources to meet identified needs. 
  • Identify and schedule additional assessments, gather collateral information as needed for processing applications and for assessments. 
  • Provide interim case management to facilitate client access to support services; take action to reduce risk to client by making appropriate referrals; develop interim case plan and work with participants to ensure goals are met. 
  • Introduce and educate families on department services and community resources, the assessment process, and participant rights. 
  • Assist and support families in the identification of needs, completion of applications, development of service goals, and the assessment process. 
  • Refer cases to cross-program teams for development of an integrated service plan and ongoing case management.

  • Good knowledge of human services principles and practices.
  • Experience in case management.
