Health Educator

POSITION SUMMARY
The Health Educator will conduct Health Promotion/Disease Prevention education on and off site when patient, staff and community health needs are identified.
ESSENTIAL DUTIES AND RESPONSIBILITIES (include but not limited to the following);
§ Coordinates the development of health cares through a comprehensive assessment using EMR/Athena and establishes priorities for the patient as a member of the interdisciplinary team.
§ Conducts individual or group health education, as well as write curriculum objectives, goals and evaluation for each educational class and program.
§ Evaluates all education materials for appropriateness.
§ Advertise and conduct prevention workshops for the community (i.e. churches, schools).
§ Customize requested health topics by age and educational background of audience.
§ Documents and maintains accurate and current patient education sheets as well as service utilization logs.
§ Assists other staff members with educational activities or events and serves as a resource. Plans month on-site health education activities.
§ Participates in community wellness program activities/events.
§ Provides monthly reports of health education activities and statistics to the Healthy Start Coordinator.
§ Contacts all patients referred for health education services by healthcare providers.
§ Utilizes the Education Department progress notes to communicate with provider on the client’s status.
§ Plans monthly on-site health education activities.
§ Assist with Community Outreach Program
a. Assist in assessing community health related needs.
b. Provide technical assistance and serves as resource on health issues.
c. Advocate for underserved population in service area.
d. Network with community agencies and organizations to promote the Healthy Start Program and services offered by TCA Health, Inc.
§ Will follow role assignments that are in alignment with the Patient Centered Medical Home Standards and participate in all Patient Centered Medical Home efforts.
§ Educates patients on use of self-monitoring medical equipment and reports results to Health Center.
§ Monitors, tracks and reports health outcomes of chronic conditions being monitored.
§ Coordinates PDSA cycles throughout the various projects; actively participates in Health Center’s Quality Improvement / Quality Assurance meetings
  • Assumes other duties as assigned by management.*
EDUCATION AND EXPERIENCE
§ Associate Degree in Nursing, Health, or Human Services, BS in Health Education preferred.
§ One to two years experience in Education, Counseling, or Human Services.
§ Certification in a disease discipline (preferred)
