Psychiatric Mental Health Nurse Practitioner

THE POSITION

Join us as we change the way health care is provided to those who need us most. Hennepin County Health Care for the Homeless is seeking a Psychiatric Mental Health Nurse Practitioner who has an interest in working with individuals experiencing homelessness. In this role, you will provide mental health care at local shelter clinics and drop-in centers and provide outreach on the street throughout the downtown Minneapolis area. You'll join a strong, diverse, multidisciplinary team of medical providers, registered nurses, social workers, licensed practical nurses and medical assistants.

New employees who are hired into remote or hybrid positions between January 2, 2022 to December 31, 2024, will receive $500 toward the cost of establishing consistent internet connectivity, payable upon completion of 6 months of employment.  

Location and hours:
This position is hybrid and will be performed both on-site at clinics in shelters and drop-in centers throughout downtown Minneapolis and remote as job duties require. This position will require travel between the different locations. Remote telehealth services will be required as well. Work hours will be Monday through Friday and will likely include one evening per week, totaling up to 40 hours per week. Clinic hours range from 8 a.m. to 8:30 p.m. No weekend, overnight, or holiday shifts are required.
 
About the position type:
This position is a limited duration (temporary) full-time, benefit earning position expected to last up to two years with the possibility of extension. 

Effective September 1, 2021, all new job offers are conditional based on the candidate being fully vaccinated for COVID-19. “Fully vaccinated” means 14 days after receiving the second dose in a two-dose vaccination series approved by the U.S. Food and Drug Administration (FDA) on a full or emergency-use basis, or 14 days after receiving a single-dose vaccine approved by the FDA on a full or emergency-use basis. After a conditional job offer is made, the candidate must show proof of full vaccination prior to their start date. A COVID-19 vaccination card from the Center for Disease Control and Prevention (CDC), a vaccination record from your doctor’s office or a personal digital vaccination record constitutes sufficient “proof.” The candidate must also attest they are vaccinated by completing a form on the first day of employment. Reasonable accommodations to this vaccination requirement will be considered upon request by a candidate for medical or religious reasons as required by applicable law.  

In this position, you will:   
Perform initial and ongoing assessments of patients' psychiatric and psychosocial status.
Provide ongoing medication management for patients including assessing effectiveness and adjusting medications for optimum effectiveness with minimal side effects; consult with other providers and team members.
Provide direct care, monitoring, and/or referral for co-occurring physical/medical conditions. 
Evaluate and treat patient with co-occurring substance use disorders, including prescribing buprenorphine.
Provide consultation to medical providers on psychiatric patient concerns.
Serve as an active team member of a multi-disciplinary team by attending meetings; actively participating in program planning, development and evaluation; and assisting with reports and special projects.
Need to have:
Graduation from an accredited school of nursing.
Possession of a valid license as a Registered Nurse issued by the state of Minnesota and a National Certification as a Nurse Practitioner (CNP), Certification in the area of specialization by an agency authorized to certify professional nurses in advanced practice OR an approved equivalent as provided under M.S. 148/284.
Access to reliable transportation to travel among shelters and drop-in centers to provide medical services.
Nice to have: 
One or more years of full-time experience working as a Psychiatric Mental Health Nurse Practitioner. 
Current CPR certification.
An X Wavier and the ability to prescribe buprenorphine.
Experience:
Providing mental health services to people experiencing homelessness.
Working with Electronic Health Records (EHR).
Sensitivity to the needs and issues of diverse and disenfranchised individuals.
Strong leadership and written and verbal communication skills.
Ability to:
Work independently and interdependently with professionals from other disciplines.
Establish and maintain effective relationships with internal and external partners.
