
Community Outreach Specialist
Hours: Full time, 40 hours per week, Monday through Friday 8:00 a.m. - 4:30 p.m. majority of the hours, with some evening and weekend hours possible. This position is grant funded and eligible for benefits.
A unique opportunity to work with high-risk families in their home and in community group settings to encourage optimal pregnancy, birth, and positive parenting outcomes. This grant funded position involves case management working with a dynamic team of public health professionals and community partners.
The home visitation program uses a nationally recognized, evidence-based model designed to prevent child abuse and neglect; Healthy Families America. Responsibilities include completing assessments of family's health, needs and strengths, coordination of services to address complex challenges, working with the family and other team members to develop a family-centered service plan, health teaching, advocacy & support. It is beneficial to have experience working with pregnant women, children, diverse populations, & families with multiple needs. Must be willing to advocate for positive, nurturing, & nonviolent discipline of children. Requires possession of a valid driver's license in the State of Nebraska. Qualifications: Graduation from a senior high school or equivalent and some experience with local, state and federal assistance programs including health care and social service systems; or any equivalent combination of training and experience that provides the desirable knowledge, abilities and skills.
Necessary Special Requirement: Possession of a valid driver's license when operating a vehicle is necessary for the satisfactory performance of assigned duties.
