Behavioral Case Manager
Under the supervision of the Behavioral Health Supervisor, this position is responsible for providing support and offering services to enhance individuals ability to thrive independently and in the community. This position provides Rule 79 case management to adults with serious and persistent mental illness. This position also provides case management services to adults under court commitment as mentally ill or chemically dependent. This position is responsible for developing and revising Functional Assessments (FA), Individual Community Support Plans (ICSP), Level of Care assessments (LOCUS), and general service plans; referring and coordinating services; monitoring the physical and mental health of clients; traveling to appointments within and outside of Blue Earth County; completing documentation and reports; monitoring court commitments; participating in discharge planning; and providing crisis assistance. This position also assists with case management intake duties, case management eligibility determinations, information and referral, and pre-petition screenings for court commitment. Additional duties include planning, initiating, coordinating and monitoring CADI waiver services for individuals with mental illness.
