Licensed Practical Nurse (LPN) Home Health- Dickinson County Healthcare System - Iron Mountain, MI
The Dickinson County Healthcare System (DCHS) seeks a full-time Licensed Practical Nurse (LPN) to join their Home Health team in Iron Mountain, Michigan.
This position works under the direction of home health professionals to provide in-home assessments and determine best course of treatment for clients with a variety of clinical needs.
The LPN reports to the Home Health Manager.
RESPONSIBILITIES.
- Comply with best practices in nursing as well as established policies and procedures to provide quality patient care or treatments (i.e. wound care, catheter care, foley insertions, bowel/ostomy care), assess needs, plan and modify care, and other nursing interventions for patients in a private home setting.
- Observe, report, document and maintain data on patient record (i.e. changes in patient condition, adverse reactions to medications/treatments, etc.).
- Maintain effective, appropriate and professional communication with healthcare team, patients and care providers at all times.
- Provide education to patients and family members on safe, proper nursing interventions and alleviate stressors.
WAGE/BENEFITS. Starting pay of $16.89 - $20.66 per hour based on years of experience. In addition, the position offers medical, dental, vision, pension plan, paid time off (sick, vacation) and life insurance.
SCHEDULE. This is a full-time, day-shift position with varying start times between 7:00am – 8:00am, or as assigned, and includes participation in the weekend work schedule rotation.
MINIMUM AND PREFERRED QUALIFICATIONS.
- Licensed Practical Nurse in the State of Michigan required; Licensed in the State of Wisconsin preferred or ability to obtain Wisconsin LPN License within 60 days of hire.
- Basic Cardiac Life Support (BCLS) certification or ability to obtain within 30 days if hired.
- Previous home health nursing experience preferred.
- Possess a valid driver’s license and maintain a good driving record. Must be willing to travel to patient locations in a hospital provided vehicle or personal vehicle during various weather/road conditions.
- Proficiency with computers and able to accurately enter patient information into an electronic medical record.
- Ability to manage a diverse workload and prioritize tasks.
- Ability to physically and mentally carry out the functions of this position.
- Exceptional customer service skills with a professional demeanor that ensures trust, competence and compassion amongst patients and team members.
- Ability to successfully pass a pre-hire drug screening and background check.
