Health Advisor
Pack Health is a digital health coaching platform for chronic disease management. We partner members with their own personal Health Advisor, who provides weekly coaching sessions and personalized follow-up online and over the phone, according to the member's communication preferences. In addition to being a virtual partner in problem-solving, coaches share videos, activities, and resources to support the member's specific health needs.
Pack Health is seeking a Health Advisor to be part of the Health Advising team. As a team member, you will have the opportunity to work with other Health Advisors to solve real problems for our members. You will be involved in all aspects of the member experience. This is a critical position for Pack Health, which is a high-growth company building a new model of delivering health care. Responsibilities:
- Coaches members and provides an empathetic, non-judgmental environment to identify effective solutions to healthcare barriers.
- Works telephonically/electronically with members to manage their chronic condition
- Interface with member profile and data within Salesforce
- Develop rapport and relationships with individuals to increase self-efficacy
- Identify and solve barriers to individual’s health management
- Provide an above and beyond experience assisting members in locating various services, physicians, or resources
- Monitor data reported directly by members to create a personalized improvement
- Use KPIs to assess work performance
- Utilize strengths and interests to work on various projects within the company
- Being a champion for quality and continuous improvement
Position Requirements:
- Bachelor’s or master’s degree in a relevant discipline (e.g. Public Health, Nutrition, Exercise science, etc.), and/or equivalent experience (e.g. allied health profession.)
- Excellent problem-solving capabilities
- Excellent teamwork skills
- Proficiency in some of the following (not all are expected):
- Customer service
- One-on-one education/coaching
- Working knowledge of healthcare systems
- Prior experience with chronic condition management preferred
- Experience with patient communication in healthcare setting
- Hunger to learn and improve through professional development, continuing education opportunities, and national health coach training
- Ownership mentality
- Immigration or work visa sponsorship will not be provided
