Spring 2019 RN/HEALTH COORDINATOR INTERNSHIP
Purpose:
To learn how to effectively and efficiently provide the management and coordination of comprehensive health care services for individuals served by AIM Services, Inc.; monitor health status (prevention, detection, etc.). Participate as an active team member, providing information, documentation, collaboration with other team members.
Status: Non-Exempt
Qualifications:
* Flexibility with schedule to meet residents needs.
* Valid NYS Drivers’ License, acceptable to Agency insurance standards is preferred. Maintain valid insurance and provide insurance card if you use your own vehicle.
* Strong organization skills with a keen ability to prioritize and multi-task. Detail oriented.
* Edits work for spelling and grammar, presents numerical data effectively, and is able to read and interpret written information.
* Excellent verbal and written communication skills.
* Adherence to Agency policies including Attendance Policy.
Physical/Cognitive Requirements:
* Ability to maintain a high level of professionalism and confidentiality. Ability to remain calm in stressful or demanding situations.
* Sitting and/or computer use/writing for extended periods of time.
* Ability to perform multiple tasks.
* Bending, reaching, pushing, and pulling.
* Lifting/transferring – individuals/items of various weights – must be able to lift a minimum of 50 pounds.

Essential Functions:
* Working with staff to maintain current knowledge of each individual's medical history and medication regimen.
* Shadow director as she performs individual nursing/health assessments, and provide and ensure appropriate follow-up and documentation.
* Observe staff as they monitor individual's health status through weekly program visits, dinner visits, ICF quarterly nursing physicals and document and follow-up as needed.
In addition, ensure timely and appropriate follow-up of all health/medical/clinical recommendations, ensure that nutritional needs of each individual are met, in consultation with a Registered Dietitian (for ICF) and/or outside sources as necessary (for IRA’s).
* Learn compliance with all state, federal and agency policies and procedures.
* Observe the accurate and timely documentation of nurse notes, quarterly nursing reviews, semi-annual and annual individual reviews, and other documentation as required/ necessary.
* Asist with tracking and review of Medical tests and follow-up as needed.
* Participate in trainings as required or requested by Supervisor.
* Shadow On-Call duty as scheduled if possible.
Other duties:
* Attend individual and agency training and updates as assigned with regards to medical/health related training, (i.e., AMAP Certification, orientation, lifting, nutrition, etc), as needed.
* Assist with back-up coverage for other Health Coordinators as needed.
* Assist in the administration of Heptavax and Mantoux, and others, as needed.
* Participate on committees as assigned/needed.
* Any and all other related duties as requested by supervisor.
* Effectively provides feedback to staff and colleagues.
