Intake Assessor (LCSW or LPC) (1099 Contractor)

Willow Oak Community Behavioral Health Center, Inc., located at 55 Jefferson Parkway Suite 55B, Newnan, GA 30263, is hiring an Intake Assessor (LCSW or LPC) (1099 Contractor). Position details: 1099 independent contractor position; Community-based role, approximately 10 hours per week; This position requires working in the office at our Newnan location at least one day per week; $60-$70 per hour; Owns each new Medicaid/CMO client's first 3-4 days of care: completes all intake documents, conducts comprehensive diagnostic assessments, makes level-of-care and treatment recommendations, and connects clients to ongoing care with Willow Oak providers and discipline-specific team members; completes all required handoffs and care coordination so ongoing providers can start treatment without gaps; Works with children, adolescents, and adults entering behavioral health services. Requirements: Active Georgia LCSW or LPC license (required); Experience with diagnostic assessments and Medicaid/CMO documentation preferred; Must pass Georgia DBHDD background check and fingerprinting (at applicant's expense); Must obtain/maintain CPR, First Aid, and CPI certification (at applicant's expense).
