Responsibilities:Provide direct care and support based on individualized service plansAssist with daily living activities, including personal care, hygiene, and meal preparationSupport clients in developing life skills and achieving personal goalsEncourage community involvement and participation in social activitiesAssist with transportation to appointments, work, school, and recreational activitiesMaintain accurate documentation and progress notesImplement behavior support strategies when necessaryEnsure a safe, positive, and respectful environment for all clientsCommunicate effectively with families, supervisors, and interdisciplinary team membersPromote independence, dignity, and self-advocacy
