About the Role

We are seeking a dedicated and enthusiastic Digital Health Navigator to join our team. The Digital Health Navigator will play a vital role in assisting residents in Northwest Wisconsin with improving digital health literacy, reducing barriers to care, strengthening patient engagement, and connecting residents to both clinical and non-clinical resources. The Digital Health Navigator primarily supports healthcare access navigation, including telehealth platforms, patient portals, online appointment scheduling, accessing medical records, understanding digital health tools, and connecting to reliable online health information. The Digital Health Navigator will work closely with community partners, including healthcare providers and public health organizations, to reduce barriers to care—particularly for rural residents, older adults, and individuals with limited digital experience. This assistance is provided primarily in-person, but may also include telephone, email, text, video chat, and other communication methods that work for the individual receiving services. We expect significant travel for this role with travel reimbursement provided. This position is expected to run through July 31, 2026, with multi-year extensions possible, dependent on funding sources.

Duties and Responsibilities include but are not limited to the following:

Community Outreach and Engagement:

  • Collaborate with community based organizations, libraries, schools, senior centers, healthcare organizations, and other community groups to identify and address the digital needs of the community.
  • Work closely with community organization staff to coordinate and participate in health digital literacy programs, workshops, and events.
  • Engage with community members to raise awareness of digital literacy programs and resources available.

Health Outreach and Community Education on Digital Resources:

  • Conduct outreach at libraries, community events, and partner locations to raise awareness of CLICK services among rural and semi-rural residents.
  • Educate residents on finding and evaluating trustworthy online health resources to strengthen digital health literacy.
  • Increase access to telehealth services through either education or device lending, including online therapy platforms.
  • Deliver individual and small group educational sessions tailored to community needs.

Digital Health Technologies Support:

  • Provide hands-on support to help residents set up, connect, and use remote health monitoring devices that support management of chronic conditions such as diabetes, cardiovascular disease, and obesity. This includes Bluetooth pairing and general education on how devices function.
  • Provide device support from a technology access perspective only. Refer participants to medical professionals and safe, reputable resources for health-specific questions.
  • Configure smartphones, tablets, and other devices for accessibility, including larger fonts, high contrast, and reduced motion settings.
  • Introduce residents to fitness and healthy lifestyle apps and show them where to find health metrics across platforms and devices.

Patient Portal and Health System Digital Navigation:

  • Assist residents in navigating healthcare patient portals, including scheduling appointments and accessing telehealth services. Build residents' confidence in using digital health services independently.
  • Coordinate device loans through CLICK's device lending library so residents without reliable technology can connect to telehealth services.
  • Collaborate with regional healthcare providers to maintain a working knowledge of their patient portals and digital systems.

Digital Resource Navigation:

  • Identify barriers affecting residents' health and well-being, and connect them to resources such as food access programs, housing support, social services, transportation assistance, and job training programs.
  • Leverage NWWIB's regional workforce development network and partner relationships to connect residents with appropriate services.
  • Assist residents in navigating online enrollment for health insurance, including Medicaid and BadgerCare. Help residents identify legitimate government websites and recognize and avoid scams during online enrollment.
  • Support enrollment in other community resources, including FoodShare, WIC, Wisconsin Shares, and Unemployment Insurance, as well as general job search assistance and other non-medical needs.

Data Collection and Reporting:

  • Complete standardized intake and needs assessments to identify healthcare access needs, digital barriers, and non-medical needs such as transportation, food, and housing (not sure if this is right place for this one)
  • Maintain accurate and timely documentation of participant assessments, services provided, referrals, follow-up activities, and outcomes in NWWIB’s case management system while protecting personally identifiable information and following organizational confidentiality policies and applicable grant requirements.
  • Prepare regular reports on program activities, impact metrics, and success stories for internal and external stakeholders.
  • Use data insights to continually evaluate and improve digital literacy initiatives and services.
