Health Navigator - Traverse City Area

Northwest Michigan Community Action Agency (NMCAA) is seeking a compassionate, organized, and community-focused team member to help individuals and families connect with healthcare, behavioral health, social services, and community resources. The Community Navigator plays a vital role in addressing barriers related to the social determinants of health by providing screenings, referrals, advocacy, and follow-up.

This position serves as a trusted resource for participants, helping them navigate access to the services needed to improve health, stability, and overall well-being. The ideal candidate is a strong communicator who is passionate about serving diverse populations and building meaningful relationships within the community.

Associate degree in Human Services, Social Work, Public Health, Counseling, Education, or a related field preferred.Two (2) years of related experience in human services, healthcare, community outreach, social services, care coordination, case management, or a related field preferred.Full time, year round. $23.49 per hour.EOE.
