Health Navigation Lead - Traverse City Area

Northwest Michigan Community Action Agency (NMCAA) is seeking a compassionate, organized, and collaborative individual to help create, oversee, and advance our Community Health Navigation Program. This position plays a vital role in helping individuals and families access critical services by ensuring high-quality referral coordination, staff development, and community partnerships throughout our service area.

Bachelor’s degree in social work, Public Health, Human Services, Healthcare Administration, Community Health, or a related field preferred.Experience supervising staff and managing community-based or human service programs.Knowledge of case management, social determinants of health, care coordination, and community resource systems.Full time, year round. $25.62 per hour.EOE.
