Waiver Support Coordinator (WSC) – Tallahassee / Northwest Florida
Premier Support Services is seeking a Waiver Support Coordinator (WSC) to serve individuals with intellectual and developmental disabilities in the Tallahassee/Northwest Florida area.
This is a flexible, largely remote opportunity for an organized, self-directed professional who is comfortable managing a caseload, meeting required timelines, and working closely with individuals, families, providers, and community partners.
Responsibilities include:
- Managing an assigned caseload of individuals receiving services through Florida’s Medicaid waiver program
- Developing, implementing, and monitoring person-centered support plans
- Coordinating services with individuals, families, providers, APD, and community resources
- Completing required monthly contacts, visits, case notes, and documentation
- Ensuring required signatures and records are completed and uploaded timely
- Conducting Pre-Support Plan and Support Plan meetings within required timelines
- Monitoring services, identifying changing needs, and following up on action items
- Maintaining compliance with documentation standards and program requirements
Minimum qualifications:
- Bachelor’s degree in Social Work, Psychology, Special Education, Human Services, or another qualifying related field
- 2+ years working with special population
- Strong communication, organization, and time-management skills
- Ability to work independently and manage multiple priorities
- Strong interest in supporting individuals with intellectual and developmental disabilities
- Comfortable using technology and electronic documentation systems
- Reliable transportation for required in-person visits within the assigned service area
