Coordinated Family Support (CFS) Coordinator

About the role

Coordinated Family Support (CFS) is a Regional Center service for adults with developmental disabilities who live with their families. As a CFS Coordinator, you are the family's steady point of contact. You help the participant and family understand, access, and coordinate the services and benefits they are eligible for, in their home, in their language, and at their pace.

We are hiring across Alameda, Contra Costa, San Francisco, and San Mateo counties, including Fremont, Hayward, Oakland, Concord, San Francisco, Daly City, San Mateo, and Redwood City.

What you will do

  • Meet with participants and families in their homes and communities
  • Help families understand and apply for benefits and community resources such as SSI, Medi-Cal, IHSS, CalFresh, housing, and transportation programs
  • Coordinate appointments, paperwork, renewals, and follow up so nothing falls through the cracks
  • Work alongside Regional Center service coordinators and other providers supporting the participant
  • Follow each participant's CFS Plan of Action and track progress toward its goals
  • Teach families how to navigate systems so they build confidence over time
  • Write clear, timely notes after every visit and keep organized records
  • Communicate with the bttr team about progress, needs, and concerns

Who we are looking for

  • Experience in the disability field is required, such as work, internship, or practicum experience supporting people with intellectual and developmental disabilities
  • Knowledge of public benefits and community resources, or experience helping people access them
  • Regional Center experience is a strong plus
  • Warm, patient, and able to build trust with families
  • Reliable and organized, with strong follow through and clear writing
  • Students and graduates in social work, human services, psychology, public health, sociology, and related fields are encouraged to apply
  • Bilingual candidates are highly valued, especially Spanish, Cantonese, Mandarin, Tagalog, Vietnamese, and ASL
  • Reliable transportation, a valid driver's license, and current car insurance and registration
  • Able to pass a DOJ and FBI Live Scan background check
  • Legally eligible to work in the United States
