Care Manager Lead

OVERVIEW: The Care Manager Lead provides case oversight, administrative, and operational leadership for the Enhanced Care Management (ECM) team while maintaining a caseload of approximately 25 ECM members. This position ensures high-quality, member-centered care coordination consistent with CalAIM ECM requirements, Sacramento Children's Home policies, and contracted Managed Care Plan expectations. This position promotes staff development, quality improvement, compliance, and program performance through regular supervision, documentation review, performance coaching, and collaboration with internal and external partners. The position balances leadership responsibilities with direct service delivery to support program capacity and maintain clinical expertise.
DUTIES:
- (50%) Leadership, Supervision & Program Oversight
- Provide individual and group supervision to Care Managers (CMs).
- Conduct regular case consultations and case supervision to ensure appropriate services, interventions, and quality care.
- Monitor staff productivity, documentation timeliness, billing compliance, and quality indicators.
- Review communimetric (CANS/ANSA) and psychometric (DASS) assessments, care plans, progress notes, risk assessments, and discharge documentation for accuracy and compliance.
- Ensure fidelity to CalAIM ECM requirements, organizational policies, and health plan contractual expectations.
- Support staff development through coaching, mentoring, performance feedback, and corrective action when needed.
- Participate in hiring, onboarding, orientation, and training of new staff.
- Provide guidance to interns, trainees, or other ECM staff as assigned.
- Assist with workload distribution and caseload balancing.
- Monitor member engagement, outreach efforts, and successful care transitions.
- Participate in quality improvement initiatives, audits, data analysis, and corrective action planning.
- Collaborate with program leadership on workflow improvements and operational planning.
- Represent the ECM program during internal meetings and external partner meetings as assigned.
- (40%) Direct Care Coordination Services
- Maintain an active ECM caseload of approximately 25 members.
- Conduct outreach, engagement, comprehensive assessments, and person-centered care planning.
- Coordinate medical, behavioral health, social service, housing, education, and community resources.
- Conduct field-based and community-based visits as appropriate.
- Facilitate interdisciplinary care coordination meetings.
- Monitor member progress toward individualized goals.
- Assist members during care transitions including hospital discharge, placement changes, and transitions between providers.
- Maintain timely documentation in accordance with organizational and health plan requirements.
- Participate in after-hours consultation or crisis response as assigned.
- (10%) Administrative Responsibilities
- Maintain accurate productivity reports and supervision documentation.
- Attend required meetings, trainings, and professional development activities.
- Assist with policy development, workflow refinement, and implementation of new initiatives.
- Participate in program evaluation and reporting activities.
- Perform other duties as assigned.
Knowledge, Skills and Abilities:
- Thorough knowledge of CalAIM Enhanced Care Management requirements.
- Strong understanding of trauma-informed, culturally responsive, and strengths-based care.
- Knowledge of Medi-Cal systems, Managed Care Plans, and Social Determinants of Health.
- Excellent leadership, coaching, and conflict resolution skills.
- Strong case assessment and care coordination abilities.
- Ability to analyze documentation for quality and compliance.
- Excellent organizational, time management, and problem-solving skills.
- Ability to manage competing priorities while maintaining attention to detail.
- Effective written and verbal communication skills.
- Ability to establish collaborative working relationships with multidisciplinary teams and community partners.
- Proficiency with Microsoft Office and electronic health record systems.
- Must be able to read, write, speak, and understand the English language.
- Able to report to work on a regular and reliable basis.
- Ability to work in an office, remotely, and in community work settings as required.
- Ability to maintain compliance with all HIPAA, confidentiality, and organizational safety requirements.
- Ability to travel locally.
- Ability to work a flexible schedule, including occasional evenings as needed.
- Ability to work with exposure to unpredictable environments during community visits.
- Ability to encounter individuals experiencing behavioral health crises, substance use, homelessness, or unsafe living conditions.
LOCATION: 4343 Williamsbourgh Dr. Sacramento, CA 95823
SCHEDULE: Monday – Friday, 8:00am-4:00pm or 9:00am-5:00pm. Evenings and weekends as needed.
SALARY: $28.53 to $32.10 per hour, to start
QUALIFICATIONS
Driving is an essential function of this job; as such, employees in this position will be required to provide their own vehicle for use in the course of employment. A valid CA driver’s license and minimum insurance coverage are required.
Must be 21 years of age or older.
Education and Experience:
A high school diploma or GED is required. Master’s or bachelor’s degree in social work, Counseling, Marriage and Family Therapy, Psychology, Nursing, Public Health, or closely related behavioral health field is preferred. Active California licensure (LCSW, LMFT, LPCC, or RN where applicable) is preferred;
Three years of experience providing care coordination, case management, or behavioral health services is preferred.
Two years of supervisory or team leadership experience is preferred. Experience with Medi-Cal, CalAIM Enhanced Care Management, Whole Person Care, Complex Case Management, or similar integrated care programs is strongly preferred. Experience working with high-acuity children, youth, families, or vulnerable populations is preferred.
