The State of Connecticut, Department of Mental Health and Addiction Services (DMHAS) is currently seeking a dedicated and experienced individual for the position of Clinical Social Worker to join our team. 
 

Connecticut Valley Hospital (CVH) in Middletown is a state-operated psychiatric hospital providing comprehensive inpatient behavioral health services to individuals with complex mental health needs. CVH brings together an interdisciplinary team of clinical, medical, nursing, rehabilitation, and support professionals who work collaboratively to provide individualized treatment and prepare patients for the next step in their recovery.   • Schedule: Full-time, 40 hours per week, 1st shift, Sunday - Thursday 8:00am - 4:30pm

  • This position is eligible for weekend differential.

THE ROLE:

You will provide discharge planning and clinical social work services to patients on an acute rehabilitation unit with a census of approximately 30 patients, including individuals with significant behavioral health needs. You will work closely with an interdisciplinary treatment team to complete psychosocial assessments, provide individual and group counseling, crisis intervention, triage, case management, supportive counseling, and treatment planning.

You will help patients identify and build upon their strengths, address barriers to recovery, connect with community-based resources, and work toward greater social and emotional stability.

A key part of the position is developing, implementing, and evaluating individualized treatment and rehabilitation plans and coordinating effective discharge plans so patients can transition to the most appropriate level of care. 

You will also collaborate with community providers and other behavioral health professionals, document clinical services, monitor treatment progress, and use your clinical judgment to help patients move toward successful community reintegration.

MINIMUM QUALIFICATIONS

Licensure as a clinical social worker in the State of Connecticut.
