CONTRACTED SPEECH-LANGUAGE PATHOLOGIST - CLINICAL FELLOW RESIDENT
1. Perform clinic and school-based speech evaluations and/ or screenings and hearing screenings as needed and/or
2. Perform speech therapy.
3. Provide documentation to Management for credentialing purposes as needed in a timely fashion.
4. Provide appropriate care as a speech language pathologist to patients as assigned.
5. Provide and maintain liability insurance.
6. Submit written documentation including evaluations, daily notes, weekly summaries and discharge summaries for clinic records to patient care coordinator as well as uploading documents to sycle.net/medifusion.
7. Report directly to CFY Supervisor and Southeastern Regional Manager.
8. Preserve and protect patient and client confidentiality in all situations and with all documentation.
9. Will arrive 30 minutes prior to each appointment.
10. Participate in monthly team meetings as scheduled.
11. Work as a team player, assisting as needed.
12. Will respect patients, partners, the director, and all other staff in the speech and hearing clinic.
13. Will be required to complete rotations in both clinics and school/daycare service teams.
14. Will be required to fill in for team members who are absent or sick.
15. Will be required to complete onboarding with School Operations Coordinator and departmental managers, complete HIPPA & Infection Control training, and complete all assigned Speech Language Pathology training prescribed by supervisors.
Clinical Fellowship Requirements CFY SLP Resident must do the following in order to receive credit for CFY per ASHA Standards:
• At least 36 weeks of full-time (35 hours per week) experience (or the equivalent part-time experience), totaling a minimum of 1260 hours. Part-time work can be completed, as long as the clinical fellow works more than 5 hours per week. Working more than 35 hours per week will not shorten the minimum requirement of 36 weeks.
• Mentoring by an individual holding ASHA certification in speech-language pathology. It is the responsibility of the clinical fellow to verify certification of the mentoring SLP , and it can be done through the online ASHA Certification Verification System or by contacting the ASHA Action Center at 1-800-498-2071.
• 2 hours of direct observation of a mentor must be performed every month for a total of no less than 18 hours of direct clinical observation by the mentor during the CFY period.
• A score of "3" or better on the core skills in the final segment of the experience, as rated by the Speech-Language Pathology Clinical Fellowship (SLPCF) mentor using the SLP Clinical Fellowship Skills Inventory (CFSI) form.
• 80% of time must be spent in direct clinical contact (assessment/diagnosis/evaluation, screening, treatment, report writing, family/client consultation, and/or counseling) related to the management of disorders that fit within the ASHA Speech and Language Pathology Scope of Practice .
• Submission of an approvable CF Report and Rating Form.
