Case Manager
Support individuals with behavioral health needs by connecting them to community resources, coordinating care, and helping them achieve individualized goals. Case Managers assess client needs, develop and monitor service plans, collaborate with treatment teams and community providers, and advocate for access to housing, healthcare, education, employment, and other essential services. Services are delivered in clients' homes, the community, the office, or via telehealth using a strengths-based, culturally responsive approach. This role emphasizes care coordination, client advocacy, documentation, and ongoing monitoring while maintaining compliance with Medicaid and organizational guidelines.
