Care Support Specialist Essential Duties:
- Provide opportunities through the day to help residents meet their goals.
- Adhere to the person-centered plans, and behavior plans as written for each resident.
- Provide quality of care and safe and healthy setting free from harm while upholding the rights of all residents. Create a space of trust and relationship with all residents.
- Transport residents and attend appointments, advocate, and obtain necessary documentation, when needed.
- Must remain awake and immediately accessible in all homes requiring 24/7 care.
- Staff in Full-Time Slots are expected to work the set schedule consistently and are permitted to pick up additional shifts with approval.
Physical Requirements
- The employee may be required to assist residents with daily activities, accompany residents to appointments and community outings, and support the implementation of individualized behavioral support plans. The employee must be able to lift, carry, push, and pull up to 50 pounds independently and greater weights with assistance or appropriate equipment when necessary.
- Work is performed in adult foster care, residential, and behavioral health treatment environments. The employee will have regular exposure to residents receiving behavioral health services and may interact with individuals exhibiting behavioral, emotional, psychiatric, developmental, or cognitive challenges. The employee must be able to maintain professionalism and effectively de-escalate challenging situations while adhering to organizational policies and training.
- The position requires frequent movement throughout assigned facilities and may involve navigating stairs, transport residents, and traveling between organizational locations as needed. The employee must be able to respond appropriately to behavioral incidents, resident crises, and emergency situations.
- The employee may be exposed to communicable illnesses, bodily fluids, environmental hazards, and other conditions associated with providing direct resident care and behavioral support services. Evening, weekend, holiday, overtime, and on-call hours may be required based on operational needs.

- Ability to assist individuals entering and exiting vehicles safely. Ability to secure wheelchairs and adaptive equipment. Ability to lift, carry, push, and pull up to organizationally established limits with or without reasonable accommodation. Ability to sit, stand, bend, walk, and perform vehicle inspections throughout the workday. Ability to safely operate transportation vehicles in varying weather and road conditions.

Key Accountability Items
- Job Performance: Maintain zero advisements for any performance related issues including – attendance, documentation, compliance, engagement, attitude and paraverbal.
- Attendance: Must be on time for all shifts and must be in attendance for shift report during shift exchange.
- Documentation: Accuracy and completeness with Daily Progress Notes (DPN), Direct Data Sheet (DDS) and any other unit documentation.
- Compliance: Fully compliant with all organizational training and updates in policies.
- Engagement: Maintaining full engagement with residents while on shift.
- Attitude: Maintain a positive attitude on the floor and with all residents and staff.
- Paraverbal: Ensure there is no poor paraverbal usage while on the floor with residents and staff.
Care Support Specialist Preferred Qualifications and Education Requirements
- A minimum education of high school diploma or GED.
- Valid Driver License and reliable transportation.
- Previous experience in healthcare field
- Experience with providing direct care to individuals with disabilities.
- Ability to accept constructive feedback.
- Ability to pass background checks as required by regulatory agencies
- Ability to follow instructions, work independently and demonstrate patience and empathy.
- Strong interpersonal and communication skills.
