Nurse Consultant

As a Nurse (Consultant):

• Collaborate closely with physicians, advanced practice providers, social workers, therapists, laboratory personnel, and other disciplines to coordinate patient-centered care.
• Function as part of a nurse and social work case management team, working jointly to address medical, psychosocial, and supportive care needs.
• Contribute to individualized care planning, patient education, and ongoing clinical assessment to support continuity of care and optimal patient outcomes.
• Provide direct patient care, including management of multidrug therapy, monitoring for adverse drug reactions, wound care, and prevention of disability.
