Social Worker Intern
Summary:
The social worker intern will work close with the AEHC Leadership Team, utilizing a variety of the skills they have learned while continuing their education. They will receive a hands-on learning experience while understanding the importance of a Social Workers position within an in-home health care agency.
Essential Duties and Responsibilities:
- Assist Program Coordinators with in home Assessments
- Assist with Intake Meetings
- Observe and participate in meetings with individuals supported, families, Care Managers, and other members of the support team.
- Assist with the findings of the Assessment and implementing programs and options that fit the client best.
- Learn about the Medicaid and Qualified Service Provider (QSP) system and how home-care services support individuals in their homes and communities.
- Completing and Filing of General Event Reporting’s, follow-ups and monthly tracking
- Support advocacy efforts on behalf of individuals while promoting dignity, choice, independence, and self-determination.
- Attend staff meetings, trainings, and other educational opportunities.
- Perform duties at the request of AEHC Leadership Team
- Other duties as assigned
Qualifications
- Currently enrolled in an accredited Social Work program.
- Must be eligible to complete an internship/practicum through their educational institution.
- Strong communication and interpersonal skills.
- Compassionate, dependable, and professional.
- Ability to maintain confidentiality.
- Strong organizational and time-management skills.
- Ability to work independently and as part of a team.
- Comfortable working with individuals from diverse backgrounds and with varying support needs.
- Basic computer and documentation skills.
- Reliable transportation may be required depending on internship activities.
Preferred Qualifications
- Interest in aging, disability services, healthcare, mental health, or community-based services.
- Previous experience working with older adults, individuals with disabilities, children, families, or vulnerable populations.
- Familiarity with community resources and social service systems.
