POSITION OVERVIEW:
The Direct Care Staff plays a vital role in the supervision and support of clients, ensuring their safety, health, and well-being. This position Involves Implementing Individual service plans and treatment plans tailored to each clients needs.
Key responsibilities Include assisting clients with daily living activities such as grooming, eating, bathing, dressing, and personal hygiene, as well as promoting independent living skills. Staff members will aid meal planning and preparation, provide transportation in compliance with state regulators, and assist with housekeeping tasks.
Additionally, the Direct Care Staff supports clients with ambulation and mobility, reinforces therapeutic counseling, manages medication administration, and carries out tasks assigned by a registered nurse. They also focus on fostering personal social adjustment, recreational activities, and behavioral skills training for individuals with intellectual disabilities, all within a safe and clean home environment.
All services must adhere to the principles of the home and community-based services (HCS) and Texas Home Living Program (TxHmL) certification with thorough documentation maintained in daily services logs.
LIVE-IN STAFF:Position requires staff to remain at the home overnightand respond to any and all emergencies. Once the staff engagesin work-related activities due to an emergency, client injury or illness, he/she will be considered on the clock. Work activity must be properly documented in service logs.
CLASSIFICATION: Part-Time/Hourly, Non-exempt
Full-Time/Non-Exempt QUALIFICATIONS:
To qualify for this position the employee must:
- Have a high school diploma, or
- Have a Certificate of High SchoolEquivalency (GED credentials); or
- Have a proficiency evaluation of experience and competence to perform job tasks including the ability to providethe required servicesas needed by the individual(s) to be served as demonstrated through a written competency-based assessment
- Have experience workingwith persons with developmental disabilities (preferred)
- Be at least 18 yearsold
- Have a valid Texasdriver’s license, a good drivingrecord, and meet the requirements of insurability as set by the company insurance agent.
- Provide at least three writtenpersonal references (letters)from people not related by
blood which:
- discuss experience in the job applying for; and
- letters of referenceindicating the individual’s ability to providea safe and healthy environment for the individual(s) to be served.
- Provide proof of current CPR training/certification for ISS Employee.
PHYSICAL ABILITY &STREGNTH REQUIREMENTS:
- Must possess the physical strength and capability to lift, support, and assist Individuals weighing between 95 to 140 pounds, or more If necessary.
- Must be physically able to Implement crisis prevention Intervention (CPI) techniques when addressing clients with behavioral challenges. This may Include the use of physical restrains during emergencies to prevent harm to the Individual or others.
- Must be physically able to run or chase after Individuals In situations that require Immediate Intervention to ensure their safety.
- Must be capable of physically assisting and transferring Individuals to and from wheelchairs, beds, hospital beds, chairs, and sofas while supporting them in activities of daily living, such as bathing, feeding, and transportation.
- Must be able to stand for extended periods, up to 8 hours or more, based on the assigned work schedule.
- Must be able to remain alert and awake throughout all assigned shifts, Including overnight shifts, In various settings such as group homes and hospitals.

JOB DUTIES:
Direct Care Providers
Shifts from 4pm-12amare responsible for the following listed but not limited to:
COMMUNICATION:
- Clients who have been supported by ISS will be followed to ensure they have arrived safely at their residence.
- Staff will perform and document in the humand chat any incidents or concerns related to clients.
- Assist client with Medication Pass (SAMS) and the performance of tasks delegated by Registered Nurse using hand over hand assistance as needed.
SAMS:
- Record in EMARS (online) after assisting with or administering medication. If online documentation is not possible, use the MARS (paper) form.
- Supervise medications without errors.
SERVICE DELIVERY LOGS:
- Will be completedelectronically using FocusedSoftware daily.
- Will need to document client’sprogress or lack of progressas indicated on Program Data on ServiceDelivery Log.
DOCUMENTATION:
- Confirm that all necessary documentation—including service delivery logs, program data, seizure forms, MARS, water temperature records, fire drill logs, incident reports, as well as bowel movement and menstrual records—is accurately completed at the end of each shift or as required.
- Responsible for submitting receipts after purchase no later than the next businessday of when items were purchased.
HOUSEKEEPING:
- Responsible for dusting, cleaning ceiling fans, sweeping, laundry, vacuuming, washing dishes and bathrooms, removing trash from bathrooms and kitchen for city pick up, cleaning microwave, stove, refrigerators (checking food expiration), storage rooms, garage, and porch when dirty, and requesting pool maintenance as needed.
- You must ensure that the home is clean at the end of their shift.
DAILY LIVING:
- Will assist clientsin bathing and toileting as needed.
- Adult briefs should be inspected for each client every two hours and changed as needed.
- Escort clients on outings to purchase items and food according to their dietary needs.
- Provide recreational and leisure training in the community while giving clients choice of their activities; make use of natural supportand community activities to facilitate social interaction, and socially valued behavior.
- Immediately report unusual client behavior to the Program Coordinator or supervisor. Any incidents requiring medical attention should be promptly reported to program nurse or 911 (if any emergency). Document and specify about behaviors on the incident or behavioral report.
- Accompany clients to the hospital and remain with them until they are either admitted or relieved, as necessary.
- Accompany clients to doctor’s appointments as needed.
- Platforms such as Focused, Humand, Effortless, and Netfirms should be utilized daily
- Ensure all safety procedures are followed daily.
- Responsible for reportingany incidents with clients and documenting on client injury illness report in focused.
REPORTING:
- Responsible for completing and turning in any firedrill that may have been scheduled during shift on focused.
- Responsible for reporting any maintenance/supplies needed on humand by end of shift.
- Perform full-body checks before and after client transport, shift changes, or receiving clients from family.
Shift 12am-8am are responsible for the following list but not limited to:
COMMUNICATION:
- Must communicate with the morningshift and report any incidentsthat were presented during the shift.
- Communicate with colleagues about the client's recent diaper change, last meal, health issues, or injuries to ensure effective care.
- Staff will perform and document in the humand chat any incidents or concerns related to clients.

- Accompany clients to hospital and stay untilthey are admitted or relieved, as necessary.
- Accompany clients to doctor’s appointments if needed.
- Platforms such as Focused, Humand, Effortless, and Netfirms should be utilized daily
- Ensure all safety procedures are followed daily.

REPORTING:

ISS Center 8am-4pm or otherwise scheduled are responsible for the followingbut not limited to:
COMMUNICATION:
- Efficiently and effectively communicate with clients,parents, guardians, family members, co-workers and supervisors. The communication must be continuous thought duration of work shift and in accordance with company’s policy and procedures.
- Maintain regular communication with the nurse about client behaviors, incidents, medication, and any matters relevant to both the client and nurse.
- Communicate with colleagues about the client's recent diaper change, last meal, health issues, or injuries to ensure effective care.
- Staff will perform and document in the human chat any incidents or concerns related to clients.
SAMS:

- Responsible for cleaningthe ISS Center and the kitchen area at the end of the shift.
HOUSE KEEPING:
- Clean ISS bathrooms, wash clothes, sweep and mop floors, disinfect after each use, maintain the refrigerator, and ensure all areas are sanitized by shift end.
JOB DUTIES IN ISS
- Gas up vans at the beginning and end of shifts if appropriate.
- Submit maintenance requestfor all vans if needed.
- Accompany clients to doctor’s appointments if needed.
- Assist clients with Supported Employment if needed.
- Follow a weeklyactivity calendar to work with the clients.
- Community integration.
- Complete Objective Trainingwith assigned clients.
- Decorate ISS for seasonal activities (Christmas, Halloween, Valentine etc.)
- Performing daily cleaning inspections of vans.
- Platforms such as Focused, Humand, Effortless, and Netfirms should be utilized daily.
DAILY LIVING:
- Will assist clientsin toileting as needed.
- Must check clientadult briefs every two hours for required changingas needed.
- Escort clients on outings to purchase items and food according to their dietary needs.
- Assist clients with meal prep and assistthem with eatingmeals.
- Provide recreational and leisure training in the community while giving clients choice of their activities; make use of natural supportand community activities to facilitate social interaction, and socially valued behavior.
- Promptly report any unusual clientbehavior to ProgramCoordinator and/or supervisor. Any incidents requiring medical attention shouldbe promptly reported to program nurse or 911 (if an emergency). Document all client behavior through an Incident/ behavioral report.

FOCUSED SOFTWARE:

TIME SHEETS:
- Ensure precise daily recording of clock-in and clock-out times in Humand to accurately document hours worked.
- OVERTIME: Rememberper the company policy and procedure manualyou must

STAFF TRAINING AND ACTIVE PARTICIPATION:
- Required to attend and participate in training activities related position:
TRANSPORTATIONDUTIES:
- Provide transportation for clients, following all Texas Department of Public Safetylaws.
- Maintain van mileagelog.
- Ensure that the vans are safe and clean, inside and out, at all times.
- Complete and submit promptly (verballyto supervisor and in-writing completing form) maintenance requests to supervisor.
- There is to be no eating, drinkingor smoking in the vans.
- Vans are not to be operated for personal use by the employee or the employee’s family.
- Employees are not permitted to transport clients in personal vehicles (unless it is required by position). Proof of VehicleInsurance must be maintained in personnel’s file.
- Transport clients to and from doctor’s appointments, as needed.
- Transport clients to and from workshop, as needed.
- Responsible and accountable for safety of self, driver-employees, passengers and vehicle while driving the van or other company vehicles.
- Perform vehicle inspections, report maintenance problemsand vehicle accidentsas
outlined in company policyand procedures.
- Provide certificate of insurance on any personalvehicle being used for company business.
- Perform pre-trip inspections of company vehiclesbefore use. Report on any suspected
maintenance problemswith the vehiclesto supervisor immediately; if vehicle is unsafe to drive, report to supervisor immediately and obtain with other parties involved in vehicle accidents.
- The use of phones while driving is not permitted. If it is necessary to use a cell phone in an emergency, drivers should pull over before doing so.
- Emergency contact cell phone numbers for client families are in the vehicle binder.
- Obey all driving guidelines to avoid citations, accidents, or investigations. If any occur, follow reporting procedures.
PROFESSIONAL DEMEANOR and PERSONAL APPEARANCE:
- Act and dress in a mature, neat and professional nature, when dealing with staff, supervisorsand outside agencypersonnel. Maintain good communication with all interested parties and document each occurrence in detail.
- Follow all dress code guidelines.
TRAINING AND CONTINUING EDUCATION:
- Mandatory to attend all in-service and continuing education/training required or indicated to

PROTECTION OF CLIENT RIGHTS: Ensure that the rights of clients are protected and always respected.
ALLEGATIONS OF ABUSE, NEGLECTAND EXPLOITATION:
Residential
- Responsible for reporting all allegations of abuse, neglect and exploitation immediately (but no later than 0ne hour after having knowledge or suspicion) to the Texas Department of Family and Protective Services (TDFPS) 1-800-647-7418. Meet all state and federal guidelines at all times when it comes to reporting abuse and neglect allegations.
ISS Center
- Responsible for reporting all allegations of abuse, neglect and exploitation immediately (but no later than 0ne hour after having knowledge or suspicion) to the HHSC Complaint and Incident 1-800-458-9858. Meet all state and federal guidelines at all times when it comes to reporting abuse and neglect allegations.
EMERGENCIES: Communicate emergencies immediately to Emergency MedicalServices or 911 as appropriate. Health related issues must be promptly reported to nurse or coordinator and supervisor. Administrative issues such as facility maintenance, vehicle repair and unforeseen weather conditions should be promptly reported to Program supervisor.
EMPLOYEEHANDBOOK: I understand that I must follow the employee handbookas it is written. It can be located In focused for review anytime.
SHIFT RESPONSIBILITIES:
PROMPTNESS: Remember you are to report to work as scheduled.
ABSENT EMPLOYEE FOR SHIFT:
If your replacement does not arrive, immediately inform your supervisor, who may ask you to stay until a substitute is assigned. No shift should go uncovered. Shift changes require supervisor approval. For emergencies, notify the supervisor at least four to five hours in advance. If you know about an emergency a day before your shift, inform the supervisor as soon as possible.
ABANDONMENT of job: At no time,you may leave home with the clientsunattended. This type of neglect occurrence will be promptly reported to law enforcement authority for investigation and prosecution.
CHANGES IN SHIFTCOVERAGE: STAFF RESPONSIBILITY: Report to work as scheduled. If for an unforeseen reason you are absent, you must communicate with Program Supervisor in advance, so proper coverage is arranged.
OTHER DUTIES: Completeother duties relatedto work position, as assigned.

Employees must not distribute Internet Viruses, Worms, Trojan Horses, or Other Destructive Activities - Distributing information regarding the creation of and sending Internet viruses, worms, Trojan horses, pinging, flooding, mail bombing, or denial of service attacks. Also, activities that disrupt the use of or interfere with the abilityof others to effectively use the network or any connected network, system, service, or equipment.
Facilitating a Violationof this Agreement - Advertising, transmitting, or otherwisemaking available any software, program, product, or service that is designed to violate this Agreement, which includes the facilitation of the means to spam, initiation of pinging, flooding, mail bombing, denial of service attacks, and piracy of software. Export Control Violations - Exporting encryption software over the Internet or otherwise, to points outside the United States. Other Illegal Activities - Engaging in activities that are determined to be illegal, including advertising, transmitting, or otherwise making available Ponzi schemes, pyramid schemes, fraudulently charging credit cards, and pirating software.
JOB DESCRIPTION:

- I understand that as a Direct Care Staff, the state and federal guidelines say that I am the person that is responsible for the health, safety, active treatment and well-being of the clients.
- FALSIFICATION: I understand that falsification of documents violates both company policy and federal law. Under no circumstances will I falsifyor ask anyone to falsifyany document. I understand that this company does not encourage, support, condone, or tolerate any type of falsification and if falsification does occur, the company will aggressively pursue criminal and/or civil charges against me.
- OVERTIME: If I am paid hourly (versus salaried employees), I understand that all overtime must be pre-approved in writing by the Supervisor BEFORE I work extra hours.
- PAYROLL DEDUCTIONS / FINAL PAYCHECK: I understand that I may not incur unauthorized expenses, and that I may be responsible for paying for such expenses that have not been preauthorized in writing from my supervisor: Any long-distance telephone charges Unauthorized cable television charges. Missing cash distributed by company to employee for clientneeds (e.g., hair care, personalcare products, etc.),or missing receipts for such distributed cash, which is then unaccounted for Unauthorized purchases using company issued credit cards or charge cards (e.g., HEB card, Wal-Mart card, etc.); purchases using company issued credit cards or charge cards that are unaccounted for (i.e., missing receipts)
Company or client propertythat I have lost or damaged (e.g.,lost pager, last cell phone, stained carpet, vehicle damage, damaged walls or furniture, etc.)
Client trust fund expenditures for which cash have been issued to me, but I am unable to account for it (i.e., missing receipts or receipts not totally the amount issued).
I understand that I will be asked to sign for all monies entrusted to me on behalf of clients, and that I may never accept monies without signing for their receipt. I understand that I must return all unused monies within a timely manner, as well as paper receiptsfor monies spent.I authorize the company to deduct from my paycheck the full amount of such monies, as indicated above, in all such cases in which I am responsible for the expense.
- I understand that any unauthorized expenses that I incur may be groundsfor disciplinary action or termination.
- I understand that I may be required to pick up my final paycheck at the regional office, and in order to receive my final paycheck, I may be required to return all company property and turn in all receipts and/orremaining monies that have been issued to me.
- CIVIL AND CRIMINAL PENALTIES: I understand that the company could hold me personally liable for any civil and criminal penalties or charges that may be brought against this facility by the state or federal agencies. I understand I must follow the policies and procedures that this company has put in place for on-the-job injuries, or the company will not be responsible for my medical bills.

In addition, I may NOT transmit via e-mail any company information such as forms, and/or any clients’ information/documentation and any type of business-related Information to my personal electronic (e-mail) account(s).
Any violation to this clause will result in the immediate termination of my position with Life Choices Unlimited, Inc. and if deemed necessary the prosecution to the fullestextent of the Law.
