Valley Community Services Board is seeking a Developmental Services Support Coordinator to join our team and provide ongoing community-based support to individuals with intellectual and/or developmental disabilities and their families throughout Staunton, Waynesboro, Augusta County, and Highland County.

As a member of a passionate and collaborative team, you'll help individuals identify and access services and supports that align with their needs, interests, and personal goals. You'll guide individuals through person-centered planning, connect them with community resources, monitor satisfaction with services, and advocate for the choices and quality of life they desire.

Starting pay of $47,618.83 - $66,378.00 per year, dependent upon education and experience.

What You'll Do

As a Developmental Services Support Coordinator, you'll partner with individuals and their support networks to identify goals, coordinate services, and promote meaningful participation in their communities. Responsibilities include:

Collaborate with individuals receiving services, their families, friends, and service providers to identify and support the individual's needs, interests, preferences, and goals.

Facilitate person-centered planning through formal and informal assessments and ongoing conversations with individuals and their support networks.

Identify appropriate community-based services and supports based on the individual's person-centered plan and shared planning documents.

Make referrals and assist individuals with applying for requested services and supports.

Advocate for the highest possible degree of choice and self-determination for individuals receiving services.

Monitor services and supports to help ensure individuals receive what they need and desire to maintain a high quality of life in their chosen community.

Partner with individuals to promote satisfaction with their services and support their ability to make informed choices.

Develop and maintain positive working relationships with individuals, families, providers, and community partners.

Provide crisis intervention and assistance as needed.

Complete timely and accurate documentation in accordance with agency policies, DBHDS, DMAS, and other applicable requirements.
