Job Summary
We are seeking a passionate and dedicated Ongoing Service Coordinator (OSC) to join our expanding dynamic team. You will guide families through the Early Intervention process by coordinating services, monitoring your caseload, and managing documentation through the EI-Hub, while communicating with families and providers by phone and secure video.
You will be the family's primary point of contact and advocate, ensuring each child's Individualized Family Service Plan (IFSP) is implemented smoothly. This position offers an exciting opportunity to make a meaningful difference in the lives of children and their families.
This is a New York State position. The role is remote to start. As the program grows, periodic in-person presence at our office will be required, along with occasional field contacts (if required by law). Candidates must reside in New York State and be able to travel to the office as needed.
We service all five boroughs, Nassau County, and Westchester County.
Skills & Qualifications
Qualifications & Credentials. Meets NYS Early Intervention Service Coordinator qualifications under 10 NYCRR 69-4.4. You qualify with any one of the following:
- A bachelor's degree in health or human services (social work, psychology, special education, early childhood, human development, nursing, sociology, or a related field); or
- Two (2) years of service coordination experience; or
- One (1) year of service coordination experience plus one (1) year working with infants and toddlers with disabilities; or
- One (1) year of service coordination experience plus an associate degree in health or human services; or
- NYS licensure or certification as a Special Educator, Speech-Language Pathologist, Occupational Therapist, Physical Therapist, or Social Worker (LMSW/LCSW)
- Completion of the NYS Introduction to Service Coordination training and NYC CPSE Transition training is required prior to case assignment. Candidates who have not yet completed these are encouraged to apply — we will support you through them.
- Relevant Experience. One (1) year of prior NYS Early Intervention Service Coordination experience preferred. Experience with infants and toddlers (ages 0–3), case management, or family/human services is a strong plus.
- Program & Systems Knowledge (Preferred). Working knowledge of the EI process and hands-on proficiency with the systems and protocols used in service coordination — EI-Hub (case management, service authorizations, activity logs, progress-report uploads), the NYC DOE Document Transfer System (DTS) for CPSE transition document transfer, CPSE transition procedures and timelines, and the applicable county-specific EI/CPSE processes (NYC, Nassau, Westchester). Familiarity with Medicaid and insurance basics.
- Coordination Implementation. Proven ability to manage active caseloads, monitor service delivery timelines, and support families across natural environments.
- Documentation Skills. Proficiency in completing timely, detailed service coordination activity logs and progress updates using digital platforms.
- Communication & Collaboration. Exceptional interpersonal and advocacy skills to support families and collaborate with providers and multidisciplinary teams. Bilingual candidates strongly encouraged — Spanish, Russian, Mandarin, Cantonese, Haitian Creole, ASL, and others.
- Remote Work Readiness. Reliable internet, a private HIPAA-appropriate workspace, and comfort using digital documentation and virtual communication tools. Reliable transportation and a valid driver's license preferred for in-office presence and occasional field contacts.
Duties and Responsibilities
- Family Support & Liaison. Serve as the family's primary point of contact — provide copies of the IFSP and amendments, obtain required parental consents, and explain family rights and the EI process.
- IFSP & Meeting Coordination. Schedule, coordinate, and participate in initial, six-month, and annual IFSP reviews, eligibility determinations, and transition meetings. Notify all interested parties of meeting dates and times, and ensure family priorities drive the plan.
- Service Monitoring. Ensure all authorized services begin within fourteen (14) calendar days of IFSP authorization and continue without gaps. Identify and resolve missed sessions, service gaps, and provider issues.
- CPSE Transition. Manage the transition from Early Intervention to Preschool Special Education (CPSE), including 120-day and 90-day notification timelines and document transfer via the NYC DOE Document Transfer System (DTS) for NYC cases, and the applicable local CPSE / school-district transition process for Nassau, Westchester, and other counties.
- Multidisciplinary Collaboration. Coordinate with treating providers (OT, PT, SLP, SI, ABA), the Early Intervention Official (EIO/EIOD), and community resources to ensure cohesive care delivery. Refer families to community and long-term supports as appropriate.
- Digital Documentation. Maintain contemporaneous, compliant service coordination activity logs and complete required monitoring in the EI-Hub and our user-friendly digital documentation app.
- Advocacy & Resource Navigation. Connect families to services, benefits, and community supports that promote their child's development and family well-being.

Education
Bachelor's degree in a health or human services field (preferred) — or as required to meet 10 NYCRR 69-4.4.
Compensation
$12.00 per 15-minute billable unit — $48.00 per billable hour.
Service coordination is paid per billable unit in accordance with New York State Early Intervention billing standards. Compensation grows directly with your caseload.
Employment Type
W-2 employee, part-time to start with the opportunity to grow. Caseload-based.
Part-time, approximately 10–20 hours per week. Availability required during weekday business hours; the role does not work in fixed blocks, as families, providers, and IFSP meetings drive the schedule.
