Service Coordinator

Position Summary

Complete service coordination activities for consumer caseload. Perform related duties as assigned and that incorporate the independent living philosophy and consumer choice/control within the service coordination operational structure.

Essential Job Functions/Duties

Service Coordinator

• Responsible for initial and on-going service coordinator education and training required to assure expected performance levels and to meet quality assurance and corporate compliance.

• Responsible for being knowledgeable of requirements, regulations, policies, procedures and all aspects related to Quality Assurance & Corporate Compliance.

• Conduct scheduled and as needed phone calls to and from consumers.

• Conduct home visits in consumer’s home.

• Complete intakes as assigned.

• Complete annual re-certifications/re-assessments as required.

• Complete MCO documentation Ex. Notes, Authorizations, Person Centered Service Plans

• Conduct tasks required by MCO in a timely manner

• Complete Weekly Units Billing Reports as required.

• Document all service coordination activities as required.

• Home Mods- DME procedures.

• Complete incident reports and investigations.

• Conduct on-going calls/contacts with PAS Providers, other Providers, Service Coordination Entities, CIL’s and contractors.

• Complete and send required notices and documentation to PAS providers, other providers, service coordination entities, CIL’s and contractors as required.

• Establish positive rapport and professional relationships with CIL’s. PAS Providers, NHT entities, other service providers, contractors, MCO, SC Entities, Etc.

• Establish positive rapport and professional relationship with consumers and consumer representatives incorporating consumer choice and independent living philosophy.

• Complete documentation and reports accurately and in a timely manner.

• Conduct activities required to assist the consumer in meeting their service needs and promoting their independence.

• Educate the consumer on options, choices and responsibilities in regard to home and community-based services.

Minimum Job Requirements

1. Have a Bachelor’s degree including or supplemented by at least 12 college level credits on sociology, social welfare, social work, psychology, gerontology or another behavioral science.

2. A combination of experience and training which adds up to 4 years of experience, and education which includes at least 12 semester hours of college-level courses in sociology, social work, social welfare, psychology or gerontology or other social science.

Experience includes: Coordinating assigned services as part of

treatment plans, teaching living skills, aiding in therapeutic activities, providing socialization opportunities for individuals.

3. Must have required training, including at a minimum: DHS Service Coordinator training, MCO specific trainings. Each service coordinator will be required to have a minimum of 40 hours of training during the first year of employment and 20 hours annually.

4. A combination of case management, service coordination and/or HCBS program participant experience.

QUALIFICATIONS: An individual must be able to perform each essential duty satisfactorily. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.

LANGUAGE SKILLS: Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports utilizing business English, spelling, grammar and punctuation. Ability to speak before groups.

REASONING ABILITY: Able to solve problems and deal with variables in situations where only limited standardization exists.

COMPUTER SKILLS: Knowledge of basic computer principles; Databases software; Internet software; Project Management software; Spreadsheet and Word Processing software.

OTHER SKILLS AND ABILITIES: Demonstrated knowledge and application of Independent Living Philosophy; Ability to travel, as needed.

PHYSICAL DEMANDS: While performing the duties of this position, the employee must be able to perform essential functions of the job with or without accommodations in order to meet the physical demands of the position.

WORK ENVIRONMENT: The noise level in the work environment is usually moderate.

General Expectations

• Submit to initial Criminal and Child Clearances and periodically as required thereafter.

• Maintain current driver license.

• Maintain adequate vehicle insurance (if applicable).

• Be able to routinely travel throughout applicable regions within ABI service area.
