Rocky Neck Village & Brookside Commons Case Manager

The New London Homeless Hospitality Center (NLHHC) seeks a part-time Rocky Neck Village and Brookside Commons (RNV & Brookside) Case Manager to work within our Intensive Services Department. The RNV & Brookside Case Manager will help participants in our Permanent Supportive Housing (PSH) Program achieve long-term housing stability and independence. PSH is designed to help formerly homeless individuals with a history of chronic homelessness—often accompanied by co-occurring challenges such as severe mental illness, substance use disorders, or physical disabilities.

Location: New London, East Lyme, and East Hartford, CT

 

Schedule: 20 hours per week; Monday-Friday 

 

Compensation: $20-22 an hour; mileage reimbursement for work-related travel


What You’ll Do:

  • Provide on-site support at two different project-based locations and conduct regular face-to-face home visits. Refer individuals to community resources and support.
  • Complete initial orientation and assessments for new participants. Compile and organize all eligibility/other required paperwork in individual case files.
  • Work effectively with assigned participants to create client-centered, individualized service plans focusing on their personal goals, and implement those plans.
  • Properly record all services and referrals in accordance with NLHHC policy.
  • Assist clients in applying for and maintaining government entitlements, disability benefits, food stamps, public assistance, and rental assistance paperwork.
  • Coach clients on lease compliance, paying rent on time, unit upkeep, neighbor relations, and reporting property maintenance needs to landlords.
  • When necessary, provide support to the Property Managers at Rocky Neck Village and Brookside Commons with completing required paperwork, resolving tenant issues, and advocating for appropriate maintenance.
  • Maintain ongoing contact with landlords to identify any issues that could impact housing stability.
  • Maintain accurate and updated case files (eligibility, assessments, releases, periodic progress reports, unit reviews, etc.) in accordance with NLHHC policy and DMHAS requirements.
  • Participate in case conferences and follow up on issues as decided by the team.
  • When necessary, provide crisis intervention support.

 

What You’ll Bring:

  • Must have a valid driver’s license; this role requires the regular use of personal and/or company vehicles
  • Interpersonal communication skills
  • Conflict resolution and problem-solving abilities
  • Knowledge of CT housing laws and practices, maintaining benefits 
  • Computer literacy
  • Understanding of documentation and file management

 

Bonus Skills:

  • Background in social services
  • Experience working with individuals with mental health and substance use disorders
  • Bilingual (English and Spanish)
