Intake Specialist
Location: Libertyville, IL (office-based with community travel)
Job Overview:
We are seeking a dedicated and compassionate Intake Specialist to join our team. This role is based out of our Libertyville office, with the majority of work conducted in the community. Intake appointments take place in client homes or public settings such as libraries or coffee shops, making this a great fit for someone who enjoys working outside of a traditional office environment.
As an Intake Specialist, you will play a key role in assessing and evaluating individuals seeking behavioral health services. You will gather clinical and personal information, complete IM+CANS assessments, review documentation, and help determine the most appropriate level of care for each client.
Key Responsibilities:
- Promote AOR’s outreach programs both internally and externally; build relationships and generate referrals
- Collect and review medical documentation, including DSM/ICD-10 codes, to support accurate diagnosis and treatment planning
- Complete IM+CANS assessments and ensure accurate, timely submission
- Track program capacity, oversee new client admissions, and manage timely and appropriate discharges into the community
- Ensure accurate, thorough, and timely documentation
- Develop and implement systems to improve department efficiency, including access to services and referral management
- Support the timely flow of new clients into AOR’s care system
- Conduct regular team and case review meetings
- Create and maintain tracking systems for required documentation
- Screen and interview clients to assess appropriateness for AOR services
- Connect clients to appropriate internal programs and external resources
- Build and maintain relationships with community partners, referral sources, and stakeholders
- Provide client-centered, trauma-informed care to individuals with severe mental illness and co-occurring substance use disorders

Qualifications:
- Meets Rule 132 credentialing requirements as a Licensed Practitioner of Healing Arts (LPHA) or Qualified Mental Health Professional (QMHP)
- Master’s degree in social work, counseling, psychology, or a closely related field
- 1–3 years of experience working with individuals diagnosed with mental illness
- Experience completing IM+CANS assessments preferred
- Valid Illinois Driver’s License and daily access to a reliable vehicle
