Hospital Liaison (Boca Raton/Bilingual English/Spanish)
Position Overview:
The Hospital Liaison conducts screenings with newly delivered mothers and newborns to determine eligibility for Healthy Beginnings services. This role promotes program goals, collaborates with hospital staff, and provides services in assigned hospital locations with scheduling flexibility.
Key Responsibilities:
- Conduct eligibility screenings and assessments in hospital settings using approved tools.
- Collaborate with Birth Registrars and hospital staff to ensure accurate and timely screenings.
- Review and validate client data, identify risk and protective factors, and assess family needs.
- Serve as a liaison between hospitals, families, and community partners, supporting referrals and access to services.
- Complete required documentation, data entry, and participate in trainings and program activities.
Qualifications:
- Bachelor’s Degree in Human Services (social work, developmental psychology, and/or early childhood education). Bachelor’s degree in a different field with a minimum of 2 years’ experience with children and families.
- A minimum of 2 years in child welfare, primary prevention services, child development or related Human Services field (and Bachelor’s degree) preferred.
- Schedule includes occasional evening and weekend hours as needed
- Bilingual English/Spanish required.
- Valid Florida’s driver’s license is required.
