Transitional Care Manager

Our Lombard branch currently holds a 4.5-star CMS rating, while our Chicago branch holds a 5-star CMS rating.

As we continue to grow, we are looking for a motivated, personable, and relationship-driven Transitional Care Manager to help strengthen our referral network, support patients during transitions of care, and expand Safe Life's presence throughout the communities we serve.

Prior healthcare marketing or business development experience is welcome but not required. We are open to candidates who are early in their careers, recent graduates, or transitioning into healthcare, provided they demonstrate strong communication skills, professionalism, initiative, and a genuine interest in building relationships and serving patients.

About the Role

The Transitional Care Manager serves as an important connection between Safe Life Home Health Care and hospitals, skilled nursing facilities, assisted living communities, physician practices, discharge planners, case managers, social workers, and other community partners.

This is a highly visible, field-based role for someone who enjoys meeting people, building professional relationships, solving problems, and representing an organization in the community.

You will help introduce Safe Life's services to healthcare partners, develop referral relationships, support patient transitions into home health care, and work closely with our clinical, intake, scheduling, and leadership teams.

For candidates who are newer to the industry, training and support will be provided to help you understand home health services, referral development, Medicare home health requirements, care transitions, and our internal processes.

This role works closely with the Administrator and Operations leadership team and provides an opportunity to directly contribute to referral growth, patient census, community partnerships, and the agency's continued expansion.

Key Responsibilities

Referral Development & Relationship Building

Develop and maintain relationships with hospitals, skilled nursing facilities, assisted living facilities, physician offices, discharge planners, case managers, social workers, and community organizations.

Conduct regular in-person visits with healthcare and community partners to introduce Safe Life's services and build long-term professional relationships.

Communicate Safe Life's clinical capabilities, service areas, patient-care model, and strong CMS quality ratings.

Identify new referral opportunities across the Lombard and Chicago service areas.

Maintain consistent follow-up with referral partners and prospective accounts.

Track referral activity, leads, follow-ups, conversions, and relationship status using the agency's CRM or referral tracking system.

Learn and understand the needs of individual referral partners and help position Safe Life as a responsive and reliable home health resource.

Transitional Care Coordination

Serve as a liaison between Safe Life and hospital, facility, and physician-office teams during patient transitions to home.

Work with intake, nursing, therapy, scheduling, and operations teams to help coordinate timely starts of care.

Help patients and families understand Safe Life's services and what to expect when transitioning into home health care.

Conduct in-person or virtual introductions with patients and families when appropriate.

Assist referral partners with questions regarding available services, home health eligibility, required documentation, and the referral process.

Help maintain effective communication between Safe Life and referring providers.

Identify barriers that could delay patient admission and coordinate with internal teams to help resolve them.

Community Outreach & Market Development

Represent Safe Life at community health fairs, senior expos, hospital forums, networking events, professional meetings, and other relevant community activities.

Build relationships with senior centers, community organizations, social workers, elder care professionals, faith-based organizations, and other local stakeholders.

Identify community partnerships, educational opportunities, sponsorships, and collaborative outreach opportunities.

Collaborate with leadership on presentations, marketing materials, community initiatives, and agency branding.

Stay informed about local healthcare needs and opportunities throughout the Chicagoland area.

Reporting & Growth Support

Provide regular updates to leadership regarding referral activity, new relationships, follow-ups, conversions, and market opportunities.

Track and evaluate referral trends to identify areas for additional outreach.

Participate in operational and leadership meetings as needed.

Help develop creative strategies for increasing community awareness and strengthening referral relationships.

Work closely with leadership to align outreach activities with staffing levels, service capacity, and patient-care goals.

Who We Are Looking For

  • A healthcare professional with experience in home health, hospice, hospitals, skilled nursing, case management, or post-acute care.
  • An experienced healthcare marketer or business development professional.
  • A recent graduate looking to build a career in healthcare management, business development, public health, healthcare administration, marketing, or a related field.
  • Someone transitioning from another customer-facing, sales, community outreach, or relationship-management role.
  • An international graduate or student with appropriate U.S. work authorization who is interested in developing a long-term career in healthcare.
  • A highly motivated individual with strong communication and relationship-building skills who is willing to learn the healthcare industry.

Qualifications

Bachelor's degree in healthcare administration, business, marketing, public health, social work, management, or a related field is preferred but not required for candidates with relevant experience or strong demonstrated potential.

Previous experience is preferred but not required.

Strong interpersonal and communication skills.

Professional, confident, and comfortable meeting new people and developing relationships.

Ability to communicate effectively with physicians, administrators, case managers, social workers, patients, families, and community partners.

Self-motivated, organized, dependable, and comfortable working independently in the field.

Willingness to learn home health operations, Medicare requirements, referral processes, and post-acute care coordination.

Comfortable using Microsoft Office and learning CRM or referral-management systems.

Valid Illinois driver's license and reliable transportation for regular field travel.

Clinical credentials or experience such as RN, LPN, MSW, or another healthcare background are a plus but are not required.
