Services and Programs Coordinator
The Neighborhood Network for Seniors is part of a network of local programs that mobilize and coordinate community volunteers and health professionals to help their older neighbors stay in the homes they love. Our services which our staff and volunteers provide fills in a large gap of need for seniors in our area. We provide reliable, personal service to these individuals, enabling them to participate more fully in the community, attend health appointments, and live independently with dignity. Our target population are seniors (65+ and older) individuals living independently in St. Paul's Highland Park, MacGroveland, Summit-University, and Union Park Planning Districts.
This NEW position works closely with our Outreach Nurse and Volunteers and Programs Director to provide support and services to clients and caregivers. This position coordinates and supports in-home services for seniors, including placing volunteers or arranging home health care/health coordination, and assisting with referrals to community resources. Assists with clients assessments, implements a care plan and delivers and/or coordinates services necessary to enable elderly persons to remain in their homes. This position is 20 hours/week. Office hours are between 8am and 5pm, M-F. This position is required to work at the office.
RESPONSIBILITIES
- Answering client inquiries and providing service referrals
- Check in calls and visits with new clients
- In-home visits with with clients and caregivers
- Support events/classes, including on-site hosting
- Providing health and service referrals
- Supporting client and caregiver care
- Coordinating services like transportation and friendly visitors
- Help clients with non-government forms
- Maintaining records, conducting background checks, and ensuring adherence to program standards.
- Acting as a liaison between volunteers, clients, families, and staff
- Community partnership outreach and relationship building
REQUIREMENTS:
- Previous experience in volunteer management, social services, and working with older adults.
- Knowledge of community health and aging-in-place services.
- Education: College degree preferred, but not required
- Technology skills - office software, social media.
- Reliable transportation

We are a small, dedicated staff collaborating to provide services, programs and education to our community. Our office is located near Macalester College.
