Measured Wellness delivers continuous, physician-directed care for patients managing complex chronic conditions. Our Care Path Program partners with skilled nursing facilities to support patients through one of the most vulnerable moments in their recovery: the transition from facility back home. Using remote patient monitoring, in-home support, and a dedicated clinical team, we help patients recover safely, avoid unnecessary hospital readmissions, and feel genuinely supported every step of the way.
About the Role
We're looking for a highly organized, compassionate, and tech-comfortable Clinical Care Coordinator to serve as the operational backbone of our Care Path Program. You'll work side-by-side with our Nurse Practitioner to help patients transition safely from skilled nursing facilities back into their homes — coordinating logistics, setting up remote monitoring devices, and serving as a trusted point of contact for patients, caregivers, and facility partners.
This hybrid role blends field work (home and facility visits) with administrative coordination — ideal for someone who thrives on variety and wants to make a tangible difference in older adults' recovery.
What You'll Do
Patient Enrollment & Facility Outreach
- Visit patients in skilled nursing facilities to explain the program and complete consent enrollment before discharge
- Coordinate with SNF social workers and case managers on discharge details and timing
Scheduling & Logistics
- Manage scheduling for home visits, telehealth appointments, and follow-ups
- Track patients through their transition to ensure timely, continuous care
Field & In-Home Support
- Set up remote patient monitoring devices (blood pressure cuffs, scales, pulse oximeters) in patients' homes
- Conduct independent home visits for device setup, troubleshooting, and onboarding
- Help patients and caregivers get comfortable with digital onboarding platforms
Remote Monitoring & Care Coordination
- Monitor incoming vitals and compliance alerts, escalating concerns to the Nurse Practitioner
- Serve as the communication link between SNFs, home health agencies, primary care providers, and the internal care team
- Handle patient-specific tasks like pharmacy coordination, billing questions, and documentation routing
Inventory & Program Management
- Maintain device inventory and coordinate timely supply orders
- Manage compassionate device retrieval and sanitization when patients complete the program
What You Bring
- Strong organizational skills and comfort managing multiple moving pieces at once
- Excellent communication skills — equally comfortable with elderly patients, family caregivers, and medical leadership
- A proactive, problem-solving mindset
- Comfort with medical technology and basic device troubleshooting
- Reliable transportation and willingness to travel to facilities and patient homes
Preferred:
- Experience in healthcare coordination, transitional care, home health, or case management
- Familiarity with EMRs such as PointClickCare or Healthie
- Experience working with elderly or medically complex patients
Physical Requirements
Frequent local travel; ability to transport and set up monitoring equipment; regular phone, computer, and documentation work.
Compensation & Schedule
This is an hourly position at $20/hour. Full-time, part-time, and PRN schedules are available depending on your availability.
