Community Psychiatric Nurse

As a Community Psychiatric Nurse, you’ll play a critical role in supporting individuals with serious mental illness and co-occurring disorders as part of our Assertive Community Treatment (ACT) team. This is a unique opportunity to provide nursing support outside of traditional hospital or clinic settings—offering 75% of care in natural environments such as client homes and community locations. You’ll assess well-being, educate clients on medication and symptom management, monitor health, and make meaningful interventions that help people stay out of crisis and move forward in recovery.

You’ll also contribute to the overall care coordination process, collaborate with fellow ACT team members, maintain accurate documentation, and provide on-call crisis support when needed. Using a trauma-informed, strengths-based, harm-reduction approach, you’ll be part of a team that ensures high-quality, person-centered care every step of the way.

What You’ll Be Doing:

Health & Wellness Support:

Conduct health and mental status assessments during client visits (in-home, community-based, or via telehealth) and evaluate for any medical or safety concerns.

Medication Education & Monitoring:

Provide psychoeducation on medications, monitor adherence and side effects, and support clients in understanding how medications relate to their recovery.

Crisis Intervention:

Use sound clinical judgment to respond to crises and emergencies, including assessing risk and coordinating further care when needed.

Skill-Building & Psychoeducation:

Help clients develop coping strategies and independent living skills that support stabilization and long-term recovery.

Service Documentation:

Accurately document services provided each day in our client management system, and maintain updated care records.

Care Collaboration:

Work with ACT team members and outside providers (primary care, psychiatry, etc.) to coordinate care and align support plans. Provide clinical guidance and direction to Licensed Practical Nurses (LPNs) in accordance with scope of practice, organizational policy, and applicable regulations.

On-Call Rotation:

Participate in a shared on-call rotation to provide phone crisis support as needed.

What We’re Looking For:

Qualifications:

• Education: Associate Degree in Nursing required; Bachelor’s or Master’s in Nursing strongly preferred.
• License: Active Illinois RN license in good standing required (or receiving within 60 days)
• Certifications: Current Basic Life Support (BLS) or Advanced Cardiac Life Support (ACLS) certification required.
• Experience: 0–2 years of experience working with individuals diagnosed with mental illness and medical comorbidities preferred—but we welcome candidates who are passionate about learning and want to make a difference.
• Driver’s License & Insurance: Valid Illinois driver’s license, access to a personal vehicle, and auto insurance with liability limits of $100,000/$300,000.
