Location: Hybrid-mostly remote in Milwaukee surrounding area. Only office time is once a week meeting

Position Summary

Senior Planning Group is seeking a compassionate, experienced, and highly organized Nurse Case Manager to join our growing team. This is a part-time, Hybrid in the Milwaukee surrounding area (once a week meeting at the office in Brookfield) position serving clients throughout Waukesha, Washington, Milwaukee, and Ozaukee Counties. The ideal candidate is a Registered Nurse Case Manager is passionate about helping seniors and their families navigate healthcare, long-term care, and community resources.

Essential Duties and Responsibilities

Conduct comprehensive client assessments. Develop and monitor individualized care plans. Coordinate care with physicians, providers, caregivers, and family members. Provide medication administration and medication management oversight as needed. Monitor client progress and recommend changes in care. Educate clients and families regarding healthcare options and resources. Assist clients in navigating Medicare, Medicaid, and long-term care systems. Maintain accurate documentation and case notes. Advocate for clients and ensure quality outcomes. Collaborate with Senior Planning Group staff and community partners.

Required Qualifications

Current Wisconsin RN license in good standing.  Minimum of two years of case management experience. Experience working with seniors preferred. Discharge planning strongly desired. Strong knowledge of Medicare, Medicaid, long-term care services, and community resources. Excellent communication, organization, and problem-solving skills. Ability to work independently in a remote setting.
