Today we are hiring a Remote, Tele-health/Virtual Nurse Practitioner (NP) role in Delaware (DE).

Job Overview
Qualifications:
Master's degree or higher
1+ years of experience in ANY of the following:
Telemedicine Care
Care Management
ANY of the following valid licenses/certifications:
Nurse Practitioner (NP) in Delaware (DE)
Family Nurse Practitioner (FNP) in Delaware (DE)
ANY of the following valid licenses/certifications:
Family Nurse Practitioner (FNP)
Nurse Practitioner (NP)
Gerontological Nurse Practitioner (GNP)
Preferred
Managed Care (1+ years)
Key Responsibilities:
● Provide comprehensive care management for medically complex Medicaid and Dual Eligible (D-SNP) populations through telehealth and remote monitoring
● Collaborate with physicians, registered nurses, social workers, therapists, caregivers, primary care providers, managed care organizations, and community partners
● Guide patients and caregivers through care transitions including hospital-to-home coordination and advance care planning discussions
● Support quality initiatives through accurate clinical documentation including HCC coding, risk adjustment, and closure of STAR and HEDIS quality gaps
● Help develop and refine care pathways, workflows, and best practices as the Delaware program expands
● Build strong relationships with patients, families, caregivers, and interdisciplinary team members to improve patient outcomes
Schedule and Shift Details
● Participate in a shared 24/7 clinical triage and support rotation to prevent avoidable emergency department visits and hospitalizations.

- $135,000.00 per year
