Connectivity and Community Health Worker (CCHW)

My Baby My Way Foundation and FAME Institute are hiring community-minded people to help California families get connected and get care, one neighborhood at a time. As a Connectivity and Community Health Worker (CCHW), you do it through two practical steps: helping people get a free or low-cost phone through the California LifeLine program, and connecting them to Community Health Worker services and local resources that fit their needs.

This is hands-on, in-person work at community sites and outdoor events. If you like being out among people, starting conversations, and ending the day knowing you helped someone, you'll feel at home here.

What You'll Do

• Run outreach at approved community sites and outdoor events across your service area
• Help eligible residents apply for California LifeLine phone and wireless benefits, activate service, and get comfortable using their new device
• Introduce Community Health Worker services and, with each person's consent, help them complete a secure resource assessment
• Connect families to healthcare, food, housing, transportation, childcare, mental health, Medi-Cal navigation, and family wellness support
• Keep accurate records and protect people's personal information at every step

What We Provide

Once you pass screening and onboarding, you earn your Community Health Worker certification through FAME Institute at no cost. Training covers community health foundations, resource navigation, trauma-informed care, cultural humility and BIPOC advocacy, HIPAA and confidentiality, California LifeLine enrollment, documentation, and Mandated Reporter certification. You're fully prepared before you ever work on your own, with team support the whole way.

What We're Looking For

• 18 or older and able to work in person at California outreach sites
• Comfortable starting conversations and giving warm, respectful service
• Dependable, self-motivated, and happy working outdoors
• Comfortable using a phone, tablet, and secure forms, and following clear privacy and documentation steps
• Reliable transportation and the ability to lift outreach supplies up to 50 pounds
• A current CHW certification or the willingness to earn ours

Experience in outreach, enrollment, public health, social services, or customer service is a plus, as is familiarity with California LifeLine or Medi-Cal. Bilingual candidates are strongly encouraged to apply, especially Spanish speakers.

Compensation

You're paid as an independent contractor in two parts: $17 to $20 for each Community Health Worker service hour you personally provide and document, plus $25 to $30 for each eligible, completed California LifeLine enrollment. Your rate and commission are confirmed in writing before you start.

The Details

This role runs through a partnership. FAME Institute contracts and pays you as a 1099 independent contractor, provides your certification and training, and issues your tax documents. My Baby My Way Foundation handles recruiting, screening, and verifying that you meet CHW requirements. As a contractor you cover your own taxes and aren't eligible for employee benefits, and you begin CHW work only after your certification, background check, and onboarding are approved.
