Physical Therapist

SoFlo Neuro delivers concierge physical therapy services in the outpatient setting, emphasizing tailored care that supports each person’s highest functional potential.

 

Role Description This is a full-time, on-site Physical Therapist role located in Palm Beach Gardens, FL. The Physical Therapist will provide neurological rehabilitation services, including comprehensive evaluations, development of individualized treatment plans, and delivery of evidence-based interventions in clinic and concierge settings. Day-to-day tasks include performing functional and mobility assessments, providing neuromuscular re-education, implementing therapeutic exercises, activities and manual therapy, educating individuals and families on home exercise programs and safety, and collaborating with an interdisciplinary team to support holistic recovery. The role also involves documenting care in accordance with professional standards, coordinating visits in home and community environments as needed, and maintaining clear, compassionate communication that aligns with each person’s unique goals.

 

Qualifications

  • Strong foundation in Medicine and Physical Therapy to assess neurological conditions and design appropriate treatment plans.
  • Experience in Rehabilitation, with emphasis on neurorehabilitation, functional mobility, and evidence-based therapeutic interventions.
  • Demonstrated commitment to high-quality Patient Care, including clear communication, empathy, and person-centered goal setting.
  • Ability to provide community-based services safely and effectively, including concierge-style visits.
  • Active Physical Therapist license in the state of Florida.
  • Doctor of Physical Therapy (DPT) or equivalent accredited degree.
  • Prior experience working with neurological populations (e.g., stroke, TBI, neurodegenerative conditions) strongly preferred.
  • Excellent documentation, time management, and organizational skills.
  • Capacity to work collaboratively within an interdisciplinary team.
  • CPR/Basic Life Support certification and familiarity with infection control and safety protocols.
