At Somerville-Cambridge Elder Services (SCES), you'll lead a multidisciplinary team supporting older adults and adults with disabilities to live safely and independently at home.

As our Nurse Manager, you'll oversee nursing services, care transitions, clinical assessments, and complex case management programs while mentoring staff, driving quality and compliance initiatives, and partnering with healthcare and community organizations across the region.

What You'll Bring
BSN and active Massachusetts RN license
3+ years of nursing experience in a community or care coordination setting
Supervisory or management experience
Passion for serving older adults and supporting complex care needs
Strong leadership, collaboration, and problem-solving skills
